Summary

The Regional Field Medical Coordinator supervises Field Medical Coordinators within their region for the ICE Health Service Corps. Responsibilities include all care for aliens in ICE custody, compliance with standards, training, and case management. The role requires leadership, clinical expertise, and travel. Must be a licensed RN or APP.

This position is only open to USPHS officers who are currently IHSC employees.

DUTIES AND RESPONSIBILITIES:

  1. Under the direct supervision of the Section Chief, the RFMC provides first-line supervision and evaluation of FMCs within his/her respective region.
  2. Serves as a rater for mid-year, annual and interim/transfer performance evaluations for FMCs in their respective regions, contributes to evaluation for RPM for respective regions through collaboration with SFPA, and advises Unit Chief and Section Chief on personnel matters for their respective region.
  3. Responsible for direct oversight of program activities and policies related to the professionals in respective regions including the conduct of all aspects of management of assigned functions related to managed care and project management; includes planning, scheduling, and assignment of work to be accomplished by FMCs in their respective region.
  4. Coordinates the announcement, hiring, review, and selection process for vacancies in respective region if needed. Reviews the needs of specific areas of responsibility collaboratively with the Field Office Director to determine the most appropriate announcement parameters in the interest in identifying maximum qualified candidates. Collaborate with the Unit Chief, respective Section Chief, and ICE/ERO to identify other locations to announce positions for hard to fill positions.
  5. Collaborates with FMCs and Regional Program Managers (RPMs) with addressing taskings, reviewing and editing responses, and following up on issues concerning FMCU, ensuring timely and appropriate responses to inquiries, site reports, iTMS taskings and corrective action plans.
  6. Serves as subject matter expert (SME) and key advisor on ICE medical standards compliance, risk management/incident reporting, QI, and, but not limited to, infectious disease public health actions.
  7. Works collaboratively with respective RPM to monitor and provide oversight of assigned non-IHSC facilities, and reviews site visit reports to ensure health care compliance using the established Quality Review Program (QRP) in accordance with the ICE National Detention Standards (NDS)/Performance-Based Detention Standards (PBNDS).
  8. In collaboration with Senior Field Training Program Officer (SFTPO), provides orientation and training for FMCs onboarding in respective region to include, but not limited to conducting site visits utilizing approved quality review program tools, pre-occupancy site reviews for compliance with the medical portion of the applicable standards. Assists the SFTPO in the development and presentation of training for FMCU staff.
  9. Responsible for collaborating with Senior Field Training Program Officer to ensure the procurement of laptop, phone, and office space if warranted is initiated and continued follow-up.
  10. Provides oversight of Medical Payment Authorization Requests (MedPARs) generated by FMCs and non-IHSC facilities for reimbursable health care services and credentialing compliance for community healthcare providers in collaboration with Health Plan and Management Unit (HPMU). Assists with ensuring MedPARs are directed to Regional Dental Consultant or Chief, Mental Health, as appropriate by the FMCs.
  11. 11. Develops, evaluates, and coordinates improvements of systems and processes affecting the integration of multiple healthcare disciplines, e.g. (process improvement, risk management, patient safety, managed care).
    12. Monitors & performs preliminary review of incidents, sentinel events, and potential litigation situations, providing documentation of corrective actions accordingly.
    13. Participates in reviewing, developing, and revising policy, and in planning/managing and evaluating processes that impact the operation of the Unit. Assists with the development, review, and revision of written procedures and guidance for the Field Medical Case Management Unit.
    14. Responsible for ensuring consistent monitoring of Illegal Aliens (IAs) with significant illnesses/potentially life-threatening conditions, injuries, or impairments. Works closely with Regional Clinical Director (RCD) and FMC on complex medical and mental health cases to determine the best plan of action related to their treatment and most appropriate placement to ensure proper delivery of medical care.
    15. Provides routine input to the Section Chief regarding FMC performance while recognizing and developing the potential of staff for the purpose of succession planning and career development progression.
    16. Assists Section Chief with investigation and resolution development for personnel actions and issues. Advises FMCs on policies and procedures impacting workload measurement.
    17. Briefs and advises Section Chief on agenda issues in preparation for activities and meetings and represents the region/program at these meetings, as needed. Coordinates various meetings, presentations; and identifies and/or devises respective support materials.

  • PHS officers must participate in IHSC TDY On-Call Schedule (ITOS.) ITOS is either an entire month or two 16-day periods a year where, depending on the needs of the agency, the officer is required to be available to serve.

PHYSICAL DEMANDS:

Education

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

  1. Incumbent must be a Registered Nurse or an Advanced Practice Provider with extensive direct patient care experience, preferably within a correctional setting such as ICE Health Services Corps (IHSC) or Bureau of Prisons (BOP) plus a minimum of 4 years of professional experience relevant to the officer's category. Of the total professional experience, officers must possess at least 1 year of experience related to management, administration, or evaluation of public health programs. Two years of case management experience and experience developing case management programs based on evidence-based practice models with IHSC/FMCU is preferred. It is the responsibility of the incumbent to fulfill the obligation(s) of their licensing or certifying body to maintain currency. This position may be required to perform clinical activities with the scope of clinical license in times of critical needs within the agency.
  2. Preferred, achieved and maintains status as a Certified Managed Care Nurse, ANCC Board Certified Nurse Case Manager, Certified Case Manager through the Commission for Case Manager Certification, Utilization Review/Utilization Management Professional, or Certified Professional in Healthcare Management as directed by IHSC, through training and other means. If incumbent does not possess any of the above, will achieve one of the above within 36 months of onboarding.
  3. Preferred, active membership in Case Management Society of America (CMSA) and MCG certification.
  4. Knowledge of case management in the government setting and healthcare industry is required.
  5. Knowledge of ICE and IHSC, program policies, guidelines, and operating procedures.
  6. Ability to exercise a high degree of independence, initiative, judgment, and follow-through on a broad range of sensitive, complex administrative tasks.
  7. Flexibility and adaptability to sudden changes in schedules and work requirements.
  8. Knowledge and skill in Microsoft Office programs to include Microsoft Word, PowerPoint, Visio, Excel, and SharePoint is preferred.
  9. Experience in reviewing, writing, and revising policies, guidelines, and procedures.

Continued Duties from above:

18. Participates, initiates, and executes special projects. Provides data collection, statistical analysis and generates reports using various computer programs used by FMCU to include, but not limited to, monthly, quarterly, and/or annual reports, as required.
19. Provides oversight of MedPARs for reimbursable health care services from jail and health authorities, law enforcement officials, health care providers/institutions, ICE officials, and other designated individuals and organizations. Performs oversight of utilization review and case management on all hospitalizations in region.
20. Ensures requests for service authorization are completed initially by establishing the eligibility of the potential recipient by utilizing ICE data systems and/or contacting ICE officials directly by FMCs.
21. Functions as secondary level reviewer to determine medical necessity and appropriateness of requested services by consulting the IHSC Medical and Dental Covered Services Plan and clinical guidelines established by IHSC Chief Medical Officer (CMO).
22. Evaluates program goals and objectives, priorities, and activities in terms of current operating activities, program progress, and changing needs and demands. Develops and monitors an implementation plan in support of overall program goals.
23. Prepare presentations for professional, academic and special interest group meetings or conferences.
24. Submits on-going activity reports to respective Section Chief and Unit Chief which include significant accomplishments, issues, problems, and workload activity.
25. Conduct site visits to local, county, state and contract facilities where ICE detainees are housed with FMCs when needed.
26. Provides coverage and support for Unit Chief, Section Chiefs, other RFMCs, and FMCs as needed (e.g., leave, TDY assignments, etc.). Assesses and ensure adequate coverage is maintained throughout the calendar year (e.g., TDY and leave absences).
27. TDY as needed to provide clinical care support at IHSC-staffed detention facilities with critical staffing needs.
28. Travel required - 20%.
29. Available after hours via cell phone or email to address urgent or emergent issues and provide updates to IHSC leadership as needed.

U.S. Public Service Commissioned Corps officers are detailed to Department of Homeland Security as described in a Memorandum of Agreement.

SUPERVISORY CONTROLS:
The RFMC works under the general supervision of the IHSC Field Medical Coordination Unit Section Chief.
Assignments are generally long-term, recurring, or broadly defined in nature. The officer independently plans and organizes their work, determines the sequence of tasks, and selects or develops appropriate methods. Assistance from subject matter experts is rarely required. Work is reviewed based on the achievement of objectives.

Guidelines may be applicable to portions of assignments but often contain inconsistencies or are only partially established. The officer exercises judgment in interpreting and adapting existing policies, regulations, precedents, and directives-such as Public Health Service (PHS) policies-for application to specific situations. This requires moderate modification, innovation, and the selection of appropriate methods from available options.

The officer evaluates outcomes and recommends updates to guidelines or program policies.
