Summary

Registered Nurse will function in support of caregivers, Veterans and family members throughout the medical center and in community settings. The incumbent will conduct interim and annual home visits to Family Caregivers approved in the program and will provide assessment via phone and clinical video telehealth. The incumbent is responsible for developing and establishing linkages and collaborate on VA caregiver support efforts with community healthcare and social service agencies.

Help

Responsibilities include conducting Veteran centered assessments with Veterans, and their caregivers in the home environment. Assessments include safety evaluation of the home and physical and psycho-social assessments of the Veteran and their caregiver. The Caregiver Support RN must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organization. Duties include but are not limited to:

  • Making independent home visits to Veterans with complex medical and psychosocial conditions.
  • Frequent driving long distances within the Bay Pines VAHCS catchment area.
  • Performing initial assessments and follow up visits for Veterans and caregivers enrolled in the Caregiver Support Program while using the nursing process, and evidence-based practice, to develop individualized referrals for the care of each Veteran.
  • Collaborating with services internal and external to the VA in order to manage the care and needs of the Veteran.
  • Providing Veteran and caregiver education with a focus on self-management, prevention, health promotion, and wellness based on the Veteran's individual goals.
  • Providing crisis intervention services, seeking to address the cause as well as the presenting complaint.
  • Coordinating family conferences and serving as a liaison to family members.
  • Manages care for Veterans assigned to the Caregiver Support Program. This includes attending interdisciplinary team meeting to discuss patient/family issues/follow-up/discharge from program/service to facilitate care coordination and disease management of Veterans identified as requiring case management intervention.
  • Collaborates with providers, other disciplines and other members to coordinate care including initiating appropriate referrals, communicating ongoing status of the Veteran and identifying barriers to care. Exploring motivational and adherence issues, evaluating support systems, and examining patterns of over and underutilization of services.
  • Applies the nursing process to systems or processes at the unit/team/work group level to improve care. Maintains current knowledge of practice standards and guidelines related to the Caregiver Support Program.
  • Provides therapies and treatments based on Nursing assessment, LIP orders, protocols. Basic cardiac life support protocols, standards of care and in compliance with facility references.
  • Demonstrates positive communication skills and behaviors that promote cooperation and teamwork with internal and external customers.
  • Other work as assigned to meet the mission and goals of the organization.
  • Incumbent must have working knowledge, and experience, in the use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.

Qualifications


To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/08/2023.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.

Preferred Experience:
  • Approximately two to three years of case management experience Long Term Care.
  • He/she has the physical and mental capability to satisfactorily perform assigned duties.
  • Maintaining a current BLS and ACLS certification is required.
  • The Care Giver Support RN must have a free and clear driver's license and have successfully passed the relevant TMS classes related to driving a government vehicle.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
