Home Based Primary Care Program (HBPC) provides primary care services to Veterans in their home in the Atchison, Leavenworth and surrounding Eastern Kansas areas. The APRN will function as the Primary Care Provider, linking the patient and the caregiver with various health services that are offered by the multi-disciplinary team and the health care system.

The APRN will function as the Primary Care Provider, linking the patient and the caregiver with various health services that are offered by the multi-disciplinary team and the health care system.
The Nurse Practitioner (NP) demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The NP shares expertise in such care to clinical trainees providing care during clinical rotations to Veterans. The NP demonstrates, promotes, creates and ensures a culturally diverse care in which the values and needs of all individuals are respected.
The NP Home Based Primary Care (HBPC) assumes primary medical responsibility for assigned HBPC patients. Duties include, but are not limited to the following:
Make patient home visits within 50 mile radius of Atchison, KS; must have valid and unrestricted drivers license.
Provide direct care through a variety of modalities including home visits, telephone and telehealth services.
Addresses care needs to Veterans and caregivers.
Identifies a patient's medical problems and defines the medical management.
Prescribes medications and treatment in accordance with their individual scope of practice or delineation of privileges.
Determines the need for consultation from subspecialties.
Determines the need for, and facilitating admission to, the VA medical facility and other transitions of care.
Participates in the development of the individual plan of care at interdisciplinary care meetings for assigned panel of patients.
Collaborate with the interdisciplinary team across services including (but not limited to): mental health, pain, whole health, education, primary care, home oxygen, laboratory services, radiology services, innovation, PMRS, complimentary/integrated health, social work, pharmacy, physical therapy, occupational therapy, surgical services, telehealth and nursing services.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Nurse Practitioners. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. [The certification must be in the specialty to which the individual is being appointed or selected.]
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: Primary Care and Home Health experience.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

Physical Requirements: Medium Lifting (35 pounds and below); Medium Carrying (35 pounds and over); Straight pulling and/or Pushing (up to 6-8 hours); Reaching above shoulder; Use of fingers; Both Hands required; Walking (up to 6-8 hours); Standing (up to 4 hours); Sitting (up to 1 hour); Kneeling; Repeated bending (up to 1 hour); Both legs required. Near vision correctable at 13" to 16"; Far vision correctable in one eye to 20/20; Hearing (aid permitted); Ability to hear whispered voice. Emotional stability. Ability for rapid mental and muscular coordination simultaneously.
