The Madison VA is looking for a Consultative Internal Medicine Physician to join our primary care team. Come join a primary care service with a passion for primary care transformation and innovation.
The Madison VA primary care service cares for approximately 30,000 patients across 8 locations in the south central Wisconsin and northern Illinois area. The Madison VA routinely scores high as a VA Best Places to work, is a leader in delivering patient-centered care, and is level 1B facility with academic affiliations with the University of Wisconsin Madison.
This is a full-time clinical position. A strong candidate desiring part time (0.6-0.9 FTE) will also be considered. The Consultative provider will work with a nurse care manager to manage internal primary care consults. Primary care providers will consult the Consultative provider for cases that require a deeper review to determine next steps for diagnosis or management of challenging clinical problems and for patients that seek a 2nd opinion. Additionally the Consultative provider will assist less experienced providers work through common internal medicine problems as these new providers become more adept at managing complexities of Veteran care. Evaluation may occur through chart review, face to face visits, video visits, and or telephone visits as appropriate. The Consultative provider will hand-off the patient to the primary care provider at the completion of the documented consult. The Consultative provider's primary clinical practice location could be located at either the Central Madison VA location or the new Madison East Clinic. Travel to Community Based Outpatient Clinics (CBOCs) within 60 miles of Madison may be required up to 25% of the time.
This is a new position, and volume of consults is yet-to-be determined. It is anticipated that the workload will include at least 10-15 consults/week and that each consult will take 2-3 hours on average. Senior general internists, hospitalists, or "cognitive specialists" such as a rheumatology or infectious disease specialists with an interest in innovation and new models of care are encouraged to apply.
VA Primary Care Providers (PCPs) manage a complex panel of patients, providing comprehensive medical care, emphasizing health promotion, disease prevention and chronic disease management. PCPs deliver care within Patient Aligned Care teams (PACT) where each patient has longitudinal relationships with their PCP and care team. PACT teams include integrated behavioral health, clinical pharmacists, dieticians, and social workers. Complementary and integrative therapies are increasing available.
