The RN Care Manager Nurse functions as team leader/charge nurse, provides leadership, which facilitates delivery of quality nursing care for a specified group of patients using the nursing process, and works effectively with all levels of staff to include collaborative efforts with other health professionals. The nurse provides leadership in improving patient care, maintaining standards of care, and directing and teaching other personnel in a variety of settings. Is adept at consideration of patient's developmental stage, which may be anywhere on the continuum of young adult to geriatric. Duties include but are not limited to:

  • He/She is expected to comply with all mandated annual review requirements, rotate to other clinical areas as assigned and take responsibility for attending in-service and continuing education activities.
  • The RN Care Manager delivers fundamental, knowledge-based care to assigned clients while developing technical competencies.
  • The RN Care Manager is expected to work independently within their scope of practice, according to policies, training and accepted VA practice.
  • The RN is expected to work in collaboration with the Licensed Practical Nurse and/or providers from the Patient Aligned Care Team (PACT) within their scope of practice, according to policies, training and accepted VA practice.
  • The function of the RN is to provide quality nursing care to Veterans.
  • The RN is expected to work as a vital team member in collaboration with the PACT team and the ancillary departments to provide excellent quality patient care.
  • The RN possesses knowledge and skills to perform registered nurse assessment, procedures, practices, and techniques at a level determined by the Qualification Standards and demonstrated competency.
  • The RN possesses the knowledge and skills necessary to: Communicate and interact appropriately and courteously with internal and external customers; Maintain confidentiality of written, electronic, and verbal patient/employee information; Work and function within a safe manner including infection control and use of equipment; Assure that patient safety is paramount in all aspects of patient care; Integrate and apply pertinent aspects of the Outpatient Clinic performance improvement program into his/her professional practice; Utilize nursing research and literature in practice; Integrate and apply aspects of the Joint Commission on Accreditation of Healthcare Organizations National Patient Safety Goals as applicable to their area of practice.
  • The RN and all personnel who are involved with the use/cleaning/reprocessing of RME are responsible for knowing what reusable instruments, equipment, medical devices or supplies are used (inventory) in their clinical area.
  • The RN must be familiar with which items are reprocessed locally (on unit) and/or which items are sent to sterile processing and/or SPD for reprocessing and/or sterilization. If items are to be transported for reprocessing, personnel must be aware of the initial decontamination steps that must be taken prior to transport of item and the appropriate method of transport.
  • All personnel who are involved in reprocessing of RME are responsible for following the local Standard Operating Procedure (SOP) drafted in accordance with the manufacturer's written recommendations.
  • SPS and Infection Prevention and Control should be consulted when clarification of written instructions are needed.
Work Schedule: Monday through Friday, 8:00 am to 4:30 pm. May include evenings, weekends, and/or Holidays.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/17/2021.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant. NOTE: See exception in subparagraph (b) below for candidates who are enrolled in a MSN Bridge Program.
  1. The Accreditation Commission for Education in Nursing (ACEN), previously known as the National League for Nursing Accrediting Commission (NLNAC) OR The Commission on Collegiate Nursing Education (CCNE).
  2. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. In these programs, students are given a certificate of professional nursing to sit for the NCLEX and should submit this certification to VA prior to appointment. Students do not earn a BSN, but receive a MSN degree upon completion of course work. A copy of the MSN transcript must be provided to VA. Bridge programs that confer a master's degree also fully meet the education requirement, even though a Bachelor's Degree is not awarded. OR
  3. In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
  • Registration: A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA.
  • English Language Proficiency: RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I: An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II: An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR An ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR A Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III: An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR An ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR A BSN with approximately 1-2 years of nursing practice/experience; OR A Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II: A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR A Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR A Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III: Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR A Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.

Physical Requirements: Physical requirements include but are not limited to the incumbent may experience long periods of standing, sitting, walking, bending, pushing and pulling. Lifting is required though mechanical lifting equipment and/or other approved patient handling aids are used to prevent the manual lifting and handling of patients/residents except when necessary, such as in a medical emergency. While performing the duties of this job, the employee is regularly required to stand and walk. The work requires long periods of standing and walking. The employee must occasionally lift and/or move up to 50lbs. Patient mental status and physical habitus will require the incumbent to be able to assist patients during transfer that is required of examinations. Specific vision abilities required by this job function include but are not limited are close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. The RN must obtain and maintain Basic Life Support (BLS) certification.

Must be able to pass all requirements according to Optional Form 178 (Certificate of Medical Examination).

Work Environment: The incumbent is in a typical office setting where patients receive medical and mental health treatment. There is a possibility of physical harm and safety precautions are necessary. The work environment involves everyday risks or discomforts, which require normal safety precautions typical of such places as offices, meeting and training rooms. The work area is adequately lighted, heated, and ventilated. Special correct ergonomic precautions must be taken with sitting and typing and can be requested through the appropriate processes. Since work requires an elevated level of interaction with patients near clinic care areas employees may be exposed to communicable diseases/viruses. Additionally, incumbent is at times confronted by angry patients or family members so special violence prevention in the workplace safety precautions are necessary and sometimes necessitates obtaining police assistance for patients in crises.

Note: Must have Basic Life Support (BLS) certification prior to assuming clinical duties. The BLS requirement is satisfied by successfully completing an American Heart Association (AHA) BLS Healthcare Provider Course or Military Training Network (MTN). BLS courses are normally offered within 1-5 days of entry on duty. No online certifications will be accepted.

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
