The Nurse III Advanced Practice Nurse Wound/Ostomy Nurse Practitioner (NP) practices in the inpatient and outpatient clinic setting. The NP is academically and clinically prepared to perform a broad spectrum of patient care services at the expert level and possesses additional training in skin, wound and ostomy care. The NP functions independently and maintains ongoing collaboration with physicians, peers and appropriate others on the healthcare team and within the community.
The NP provides direct and indirect patient care the includes consultations, assessments, diagnosis, education, planning, and treatments of wounds, skin and ostomies. The NP provides ongoing management of patients with actual or potential skin breakdown. The Wound/Ostomy NP will serve as the facility consultant for the use of specialty care beds, wound care products, and coordinate any trials of wound care treatments.
The NP will develop educational programs related to wound and skin care, chair the facility skin care team (SWAT: skin wound assessment team), make appropriate recommendations to facility practitioners, and maintain trending data for quality improvement opportunities. The NP may initiate consults and referrals to other health care team members, prescribe medications/supplies and therapies in accordance with VA policy, VA formulary, State regulations and scope of practice. The NP role also includes research in addition to clinical practice.
The Nurse Practitioner (Wound-Care) requires VA Approved Wound Care Certification.
