The RN Care Manager is self-directed in assessing, planning, implementing, coordinating, monitoring, and evaluating patient care activities. This RN demonstrates leadership and mastery of practice in delivering and improving efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication and coordination of resources. She/he works in partnership with an assigned panel of patients and Patient Aligned Care Team (PACT) Teamlet members for effective disease and chronic care management. She/he demonstrates leadership, experience, and creative approaches to management of complex client care. The RN Care Manager is directly supervised by the Primary Care Chief Nurse or Nurse Manager assigned to Women's Health.
Responsibilities include but are not limited to the following:
Utilizing the nursing process and evidenced-based practice, the RN Care Manager collaborates with and leads his/her assigned PACT Teamlet in the development and implementation of the patient driven, holistic care plan for the veteran.
Develops the plan for health education and assures the patient and his/her family receives the information needed focusing on self-management, prevention, and wellness, based on the patient's goals.
Collaborates with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. Incumbent will work with members of the expanded health care team and consultants as the patient's goals dictate.
Provides education, management and monitoring of chronic and acute disease processes and chronic pain.
Utilizes Telehealth modalities to deliver patient care.
Makes assignments and provides direction to the other members of the nursing staff.
Serves as a preceptor and mentor to colleagues and students, enhancing the professional growth, personal development and practice of others.
Demonstrates leadership by active involvement in area-based councils, committees, and task forces.
Administers medications and procedures per established policies, protocols and guidelines. She/he considers all characteristics of the individual, including sex and sex role identification, age and life stage, state of health, race and culture, socio-economic status, values and previous experience with similar events, in order to assist each patient to identify and meet basic health needs and improve his/her health status to the individual's maximum potential.
Responsible for demonstrating and maintaining full competency in the use, pre-cleaning and/or reprocessing of reusable medical equipment. The employee must ensure that the appropriate Standard Operating Procedure (SOP) and manufacturer's instructions are followed for any step in the use of RME (pre-cleaning, reprocessing, disinfection, sterilization, maintenance) in which he/he is involved.

Qualifications

Basic Requirements:
United States Citizenship.
English Language Proficiency. RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Education: Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Licensure: Must hold a current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.

Physical Requirements: The population of male and female veterans served ranges from adult, age 18, to the elderly adult, age 65 and over. The VA Medical center is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continues walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

This RN position is temporary, not to exceed 13 months and does not confer permanent status as a VA employee for purposes of continued employment. Once the 13 months is over, your employment with the Cincinnati VA Medical Center will end unless additional funding and or approval is granted to extend your appointment / position. If additional funds/approval is approved, this appointment may be extended or possibly converted to permanent without further competition. Since this position is a temporary one, it is not a guarantee that it will be available for the entire 13 months. If selected for this temporary position, your employment with the VA can end at any time.
Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
