Summary

The Home Based Primary Care (HBPC) Program provides primary care to the most vulnerable home-bound veteran population. The SFVA HBPC covers both rural and urban areas of California within San Mateo, San Francisco, Marin, Sonoma, Mendocino, and Humboldt counties. The Nurse Practitioner (NP) provides medical assessment and support, crisis evaluation, intervention, and consultation and referral for the geriatric veterans of all ethnicities and gender.

Intermittent/Per-Diem Nurse Practitioner (Home-Based Primary Care)- Santa Rosa major duties and responsibilities include, but are not limited to:

  • Demonstrates performance and leadership that is broad enough to improve the care for a group of patients.
  • Executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex patient care.
  • Demonstrates the following:
    1. leads and organizes the delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning;
    2. Uses advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making, and evaluating outcomes;
    3. Functions as an expert in clinical practice and/or areas related to the assigned roles and responsibilities;
    4. Systematically evaluates current practice, and formulating outcomes for groups of patients and/or organizational processes within an area of expertise;
    5. Guides, develops, and supports staff from a leadership perspective; and
    6. Uses professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities.
  • Communicates with the veterans and their families to assure understanding of their disease process and the goals of therapy.
  • Collaborates with other healthcare professionals and colleagues, team building activities.
  • Guides and supports staff development from a leadership perspective, using scientific evidence and practice to evaluate programs and/or services and their effectiveness.
  • Participates in continuing education and achieving and maintaining certification sets a high standard for professional achievement.
  • Ensures that patient data is updated and accurate to ensure continuity of care. Program or service level outcomes must be documented as well, and may be broad and complex.
  • Uses advanced clinical knowledge/judgment to promote staff involvement in planning, decision-making and evaluating outcomes, contributes to organizational success as they improve patient care.
Work Schedule: Intermittent/Per Diem, Monday to Friday, 8:00 AM- 4:30 PM, with extended hours as necessary to provide patient needs. No weekends or holidays.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a nurse practitioner or otherwise recognized as a nurse practitioner in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Maintain a full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body in either case management or ambulatory care.
Preferred Experience:
  • Must have a Master's in Nursing degree.
  • Must have current and valid driver's license.
  • Must have current Basic Life Support Certificate.
  • Must have at least 2-3 years of Home Base Primary Care or Community Home Care/Hospice experience.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
  • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
  • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Physical Requirements: The physical requirements of this position include lifting, prolonged standing, frequent bending and stooping, walking, reaching, and repetitive motion. Heavy Lifting (45lbs and over); Heavy Carrying (45lbs and over); Straight Pulling (up to 1 hour); Pushing (1 - 2 hours); Reaching above shoulder; Use of fingers; Both hands required; Walking (up to 3 hours); Standing (up to 3 hours); Repeated bending (up to 1 hour); Ability for rapid mental and muscular coordination simultaneously; Near vision correctable at 13" to 16"; Hearing (aid permitted). The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing, keyboarding, reading, and logical thinking. The environment's pace can vary from steady to a fast pace requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in noise levels in the environment. There may be the need to use personal protective equipment to prevent exposure to disease, illness and hazardous materials.

Additional information

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
