This position is the Nurse (Care Coordinator) at the Woodrow Wilson Keeble Memorial Health Care Center located in Sisseton, South Dakota. Incumbent reports to the Supervisory Nurse Case Manager.

This position is the Nurse (Care Coordinator) at the Woodrow Wilson Keeble Memorial Health Care Center located in Sisseton, South Dakota. Incumbent reports to the Supervisory Nurse Case Manager.

Collaborate with Outpatient Clinic Team Members; primary care providers, clinic nurses, public health nursing, pharmacists, case management, Dietician, Physical Therapy, and social services to ensure that the patients care needs are done in a timely manner to maximize each visit. Ensure that health care maintenance is up to date for each patient.
Manages all clinic visits and follow-up visits through coordination with multi-disciplinary teams of physicians, nurse practioners, physician assistants, nursing personnel, social workers, physical therapists, pharmacists, etc.. Documents patient care coordination in the electronic health record (EHR).
Manages the team provider's panel.
Coordinates patient care with Case Managers when needed.
Huddles each morning with the care team to discuss scheduled patients and delegates assignments to other nursing team members.

MINIMUM QUALIFICATIONS for GS-09:
Your resume must demonstrate at least one (1) full year of professional nursing experience (equivalent to the GS-07 grade level or above) and possession of a diploma, associate degree, or Bachelor of Science degree in Nursing from a profession nursing program. Examples of experience include: Assess the patient care needs; adjusts staff and collaborate with the Clinical Nurse Supervisor as necessary. Develop a plan of care for individual patients utilizing nursing diagnosis. Consult with primary care providers and other healthcare professionals on patients to coordinate diagnostic and therapeutic regimens for patients. Provide orientation to new employees. Participate and contribute to clinic Performance Improvement activities. Documents observations, nursing interventions, therapeutic measures administered and patient response to treatment or nursing care. Respond to all Code Blues. Evaluate professional growth and activities implemented to improve Nursing Services.

Possession of a master's degree in professional nursing.
