Summary

Outpatient Registered Nurses (RN) provide comprehensive nursing care to patients across the age spectrum and presenting with a wide range of conditions in outpatient primary care settings. RNs assess, plan, implement and evaluate nursing care consistent with patient-centered medical home principles and emphasizing interdisciplinary collaborative processes. This is a Direct Hire Open Continuous Announcement for Outpatient RNs at multiples facilities in the Phoenix Area Indian Health Service.

Help

  • Performs comprehensive professional nursing duties for the care of patients with a variety of injuries and illnesses in the outpatient department and or field clinics. Assesses, plans, implements, evaluates and documents comprehensive nursing care for patients presenting with a wide range of health conditions and across the age spectrum in collaboration with the patient, family, caregiver(s) and interdisciplinary team, and aligned clinical practice guidelines and standards of care.
  • Administers medications and therapeutic treatment measures and care procedures per established policies and guidelines; observes and documents patient condition, changes and reactions to medications and medical treatment(s).
  • Uses appropriate medical supplies and/or equipment, such as 12-lead EKGs, fetal monitor, pulse oximeter, blood glucose meters, nebulizers, monitoring devices (cardiac monitors, oxygen monitors, etc.), etc.
  • Closely observes patients for conditions that may require emergent measures; recognizes, identifies, and interprets serious clinical situations; provides appropriate immediate emergency measures.
  • Coordinates interdisciplinary patient care and support activities across the healthcare continuum. Plans for care management of targeted patient populations; assures pre-visit planning for patients; coordinates internal and external care referrals, develops and/or maintains disease and case management registries. Assesses patient's needs and health status data, individually and/or with the care team, to develop a plan of care.
  • Accepts responsibilities as team leader and/or charge nurse. Coordinates clinical and patient flow, prepares assignments and provides guidance and leadership to nursing personnel of varied skill levels. In the absence of the supervisor, accepts those supervisory and administrative responsibilities.

Works on a regularly assigned rotation to provide continuity of medical service 24/7 including weekends and holidays. On-call hours or standby duty may be required.

May drive a U.S. Government vehicle and must have a valid, unrestricted driver's license.

Selectees are required to have a bank account at a financial institution for electronic direct deposit of salary payments.

Measles and Rubella immunization required for selectees born after 1957. Seasonal Influenza immunization is required for civilian health care facility staff working in Indian Health Service health care facilities.

The selectee is required to obtain and maintain medical staff clinical privileges, including any licensure requirements. If privileges are not obtained or maintained during employment, the employee will be subject to adverse actions, up to and including removal from Federal service.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week.

For positions requiring positive education requirements, or if you are using education to meet all or part of the qualification requirements, you MUST submit a copy of your transcripts or an itemized list of college courses which includes equivalent information from the transcript (course title, semester/quarter hours, and grade/degree earned) in your resume.

BASIC REQUIREMENT(S): All applicants must meet all the basic requirements:

  1. Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
  2. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
  3. Licensure: For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Per The Indian Health
    Manual, Chapter 1, Chapter 3 - Verification: Members of the medical staff and others who must apply for clinical privileges must hold an active and unrestricted State license, certification, or registration, as applicable, to practice in their professional field. The term unrestricted means that there are no restrictions, special considerations, periods of monitoring, or probationary requirements associated with license, certification, or registration that restricts or inhibits the ability of the practitioner to practice his/her profession in the specialty or clinical area for which the practitioner is being hired. This includes any stipulations that may have a potentially significant adverse impact on patients, the medical staff, or the efficiency of the facility. In general, providers with any restrictions on any State license, certification, or registration will not be granted medical staff membership or clinical privileges.
Experience: The work experience must have equipped the applicant with the particular knowledge, skills and abilities to successfully perform the duties of the position. At the GS-9 and above grade level, many positions may require experience in a specialty area of nursing.

Basic Requirements for the GS-5 (or equivalent) Grade Level
To qualify at the GS-5 (or equivalent) grade level, applicants must meet one of the following qualification requirements:
  • Possession of a bachelor's degree in Nursing with no experience; or
  • Possession of a diploma or associate degree, in professional nursing and one full year of professional nursing experience.
Professional Nursing Experience is obtained in either the private or public sector performing the following type of work and/or tasks:
  • Experience in assessing clinical conditions, planning, implementing and evaluating care for a variety of patients based on age-specific components.
Basic Requirements for the GS-7 (or equivalent) Grade Level
In addition to the above Basic Requirements, applicants must meet one of the following requirements:
  • Possession of a bachelor's degree in Nursing and eligibility under the Superior Academic Achievement provision;
  • Possession of a bachelor's degree in Nursing and at least one (1) full year of professional nursing experience (equivalent to the GS-5 grade level)
  • Possession of a diploma or associate degree in professional nursing and two full years of professional nursing experience; or
  • Completion of at least one full year of graduate education in a professional nursing educational program.
Professional Nursing Experience is obtained in either the private or public sector performing the following type of work and/or tasks:
  • Experience in assessing clinical conditions, planning, implementing and evaluating care for a variety of patients based on age-specific components.

Nurse (Outpatient) GS-09 Grade Level:
  • Possession of a master's degree in professional nursing; or
  • At least one full year of professional nursing experience (equivalent to the GS-7 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program.
Professional Nursing Experience is obtained in either the private or public sector performing the following type of work and/or tasks:
  • Experience at independently assessing clinical conditions, planning, implementing and evaluating care for a variety of patients based on age-specific components.
  • Experience providing guidance, direction in specific task and job techniques to clinical and nonclinical staff and coordinating clinical flow within the clinical care team including planning, allocation and adjustment of resources in response to workload and priority changes.

Clinical Care Coordinator GS-10 Grade Level:
  • Possession of a master's degree in professional nursing; or
  • At least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree in Nursing from a professional nursing program.
Professional Nursing Experience is obtained in either the private or public sector performing the following type of work and/or tasks:
  • Experience in the management of nursing care in outpatient primary care environments and/or case management, emphasizing interdisciplinary collaborative processes to assess, advocate, plan, implement, coordinate, monitor and evaluate health care options and services which are consistent with patient-centered medical home principles.
  • Experience in coordination of patient care and resources for a caseload of patients across the continuum of care and through care transitions; able to provide patient and family education pertinent to a wide range of conditions and health factors; skilled in supporting patient self-management.
Basic Life Support (BLS) certification is required for all Outpatient and Clinical Care Coordinator Registered Nurse positions.
