The Sheridan VA Health Care System is recruiting for one (1) Clinical Pharmacy Specialist (PACT) , GS-0660-13, Pharmacy Service position to serve as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This position will support the mission at the Gillette Community Based Clinic, Gillette, WY.

Manages as a mid-level provider within a scope of practice in providing care for a wide variety of disease states; independent practitioner with maximal prescribing privileges and autonomy under an approved scope of practice.
Achieves positive patient centric outcomes through direct and indirect interactions with patients.
Designs, implements, assesses, monitors, and documents therapeutic plans based on patient-specific information utilizing the most cost effective, least toxic, and most effective therapy.
Completes an appropriate review of all medication orders to include selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitors for efficacy, side effects and clinical outcome through selection and evaluation of physical and laboratory parameters; and advises prescribers as appropriate.
Reviews all medication orders for appropriateness, drug selection, dosage, route of administration and the amount, and adherence to the VA formulary process.
