This is an indefinite position that may convert to permanent without further competition.

NATIONAL GUARD MEMBERSHIP IS REQUIRED:
This is an excepted position that requires membership in a compatible military assignment in the Connecticut Army National Guard.

JOB SUMMARY: The primary purpose of the position is to facilitate medical readiness and medical retention for National Guard military members. Using a collaborative team approach, assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet complex health needs of Soldiers in order to promote health, wellness, and deployability. Addresses patterns or problems at the patient, provider, and/or systems level to promote smooth transition of Soldiers through the Military health care experience. Seeks resolution for identified problems, through individual or collaborative efforts to assure appropriate utilization and optimization of resources Position provides non-clinical case management to provide pre-mobilization and post-mobilization Individual Medical Readiness (IMR) support to include identification and tracking of Service Members (SM) who are not Fully Medically Ready (FMR) either for dental or medical concerns. Cases will be obtained from Annual Periodic Health Assessments (PHA) events, Home Station (HS) or Mobilization Stations (MOBSTA) SM Readiness Processing (SRP) events or identified for those SM determined non- medically or nonĀ­-dentally ready. Case Managers shall assist SMs to become Fully Medically Ready (FMR) by assisting with tracking medical and dental appointments, ensuring medical compliancy by maintaining communications as required with the SM they are managing.

The major duties include, but are not limited to:

  • Interpret and translate SM limitations from non-military providers.
  • Provides advice, guidance, and health teaching to SM and Chain of Command, in individual or group settings designed to assist the SM in understanding, accepting and following medical recommendations.
  • Coordinates and implements health prevention and education programs related to fitness for duty, medical readiness, and deployability.
  • Assumes responsibility and accountability for case management of all Soldiers in case load.
  • Assists in ongoing patientcare coordination until planned outcomes are achieved.

  • Must be HIPPA trained and maintain certification.
  • May occasionally be required to work other than normal duty hours; overtime or compensatory time maybe required.
  • Must maintain a current license as a Registered Nurse.
  • This position requires a bachelors with an RN or an RN with a certificate and 2 years of experience, or an LPN with minimum of 2 years of experience, and active state licensure or transferable license from other states.
  • Must be able to obtain and maintain a secret security clearance.

Qualifications

MANDATORY EDUCATION & LICENSURE REQUIREMENTS: Applicants must have passed the National Council Licensure Examination -AND- possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Applicants must possess a graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery: Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. (Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign. Nursing Schools is required for individuals who graduated from foreign nursing schools.)

GENERAL EXPERIENCE: Applicants must possess a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; -OR- at least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program.

SPECIALIZED EXPERIENCE: Your resume and any other supporting documentation you wish to provide must clearly substantiate you currently possess 6 months of specialized experience in the following:

  • Experience demonstrating knowledge of the basic principles and practices related to the management of occupational health care delivery systems.
  • Experience planning, developing, analyzing, and briefing the occupational health nursing services provided to National Guard personnel throughout the state.

KNOWLEDGE, SKILLS AND ABILITIES:
  • Knowledge of current standards of practice for utilization management as described by the Department of Defense, Department of Army, U.S. Army Medical Command, National Guard, State policies and current literature.
  • Ability to administer the case management of National Guard Service members to include identifying necessary changes, advising others of its requirements and acting as liaison in providing information and education on the program.
  • Knowledge and skill in applying analytical and evaluative methods and techniques to review in order to identify case management issues, determine medical readiness of service members and make recommendations for improvement.
  • Knowledge of nursing theory and practice with current clinical hands-on experience to evaluate, assess, and recommend the appropriate service member healthcare plans.
  • Knowledge and ability to use professional nursing clinical expertise to assess, evaluate and recommend case management interventions.
  • Ability to communicate effectively in order to gather data, present information, coordinate and facilitate activities, and make recommendations both orally and in writing.
  • Knowledge of techniques for data analysis to identify patterns, trends, or issues of significance, and the ability to analyze the data, draw conclusions, and present data.
  • Knowledge of medical automation and its use in data collection and display.

Education

No substitution of education for specialized experience.

FOREIGN EDUCATION: Education completed in foreign colleges or universities may be used to meet the requirements. You must show proof the education credentials have been deemed to be at least equivalent to that gained in conventional U.S. education program. It is your responsibility to provide such evidence when applying.

Additional information

COMPATIBLE MILITARY ASSIGNMENT: All applications will be considered regardless of Branch/AOC/FA/MOS. Incumbent will be required to be assigned to compatible military position within 1 year of selection. The selected individual may be required to attend formal technician school training.

UNIT OF ASSIGNMENT: Any unit within the state

SPECIAL REQUIREMENTS: For positions requiring the operation of motor vehicles, candidates must have a valid state driver's license for the state in which they live or are principally employed. This position requires a security clearance (secret) which the applicant must possess or be able to obtain a clearance within one year of appointment.

CONDITION OF EMPLOYMENT: Training at the National Guard Professional Education Center (NGPEC) is a mandatory condition of employment, subject to availability of resources. NGB prescribed courses at NGPEC must be completed within 6 months of assignment to this position. Failure to complete course(s) may result in reassignment or termination of full-time employment unless extenuating circumstances preclude course attendance.

Primary Change of Station (PCS) relocation expenses will NOT be paid.

To ensure compliance with an applicable preliminary nationwide injunction, which may be supplemented, modified, or vacated, depending on the course of ongoing litigation, the Federal Government will take no action to implement or enforce the COVID-19 vaccination requirement pursuant to Executive Order 14043 on Requiring Coronavirus Disease 2019 Vaccination for Federal Employees. Therefore, to the extent a Federal job announcement includes the requirement that applicants must be fully vaccinated against COVID-19 pursuant to E.O. 14043, that requirement does not currently apply.
