Summary

The Clinical Cardiac APRN executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care distinguished by autonomy of practice. APRN's exercise a high degree of independent judgment, initiating care plans, and treatment regimens for Cardiology patients. Practice is characterized by an increased complexity of clinical decision making and management under the supervision of collaborating physicians.

Help

  1. Meets all mandatory requirements for assigned area and will perform activities that reflect the educational, experiential, and competency requirements.
    1. Within the APRN scope of practice, and under the supervision of the cardiologist, diagnoses medical condition, plans and implements interventions, and evaluates veteran response in the outpatient and inpatient settings.
    2. Within the APRN scope of practice, assembles and documents complete Veteran medical database including health history and physical examination.
    3. Demonstrates leadership in the education and support of the veteran, family/significant other and health care team members.
    4. Functions as a bridge between the veteran and health care teams; communicates with the referring teams, including providers and nurses, to ensure the veteran's needs are addressed.
    5. Provides outreach to patients and families of patients with life limiting illness. Includes explaining services and roles involved with Cardiology, providing psychosocial support and addressing concerns about medical care and symptom management. This includes educating the staff at the VA about Cardiology issues and initiatives.
    6. Facilitates communication between providers. Includes working with the veteran's care team, attending departmental meetings, developing working relationships with multiple departments, and ensuring patient and family needs are identified and addressed in a timely manner.
    7. On an ongoing basis, coordinates appointments, reviews medical records and ensures the veteran's Cardiology needs are met. Also ensures timely follow-up appointments are made.
    8. Provides education for veterans and families. May include addressing questions regarding procedures, treatments, symptoms, and terminology; and discussing, explaining and reviewing recommendations of the Cardiology Team.
    9. Provides psychosocial support. Includes regularly assessing the state of mind of veteran and family, addressing identified problems, using motivational interviewing, problem solving, and assisting with necessary consults as appropriate to other services (i.e. Mental Health, Social Work, Chaplain, Pharmacy, and Dietary).
    10. Within the APRN scope of practice, monitors and manages symptoms. Includes identifying symptoms and working with patients, providers and resources to manage them appropriately. This may include prescribing medications and ordering additional testing or consultations.
    11. Identifies and recommends resources. Shares knowledge of helpful resources for patients, including local support groups and classes; assists veterans and families with accessing these resources.
    12. Documents coordination of care provided in CPRS and other appropriate resources; manages alerts in CPRS.
    13. Request and send pertinent medical records/reports.
    14. Assist in gathering data necessary for facility and VISN reporting.
    15. Identifies potential emergency/crisis situations and provides appropriate interventions.
    16. Seeks out and participates on committees and task forces as needed/appropriate.
    17. Participates in program and clinic activities and meetings.
    18. Provide educational opportunities to the VA Staff and community at large about topics specifically related to nursing care for patients with Cardiology needs. Educational offerings should be available in a variety of settings (nursing huddles, facility wide symposiums, and other settings as needed).

Work Schedule: Monday - Friday 8am to 4:30pm

Qualifications


Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Licensure - Current licensure as Advance Practice Nurse Practitioner.
Preferred Experience:
  • MSN with ANCC certification or equivalent with clinical experience in Cardiology, Heart Failure or Clinical Cardiac Electrophysiology Preferred.
  • Minimum three years nursing experience in Critical Care or minimum one year nurse practitioner experience in the last 24 months.

Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
  • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
  • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Physical Requirements: This position involves considerable amount of standing, walking, stooping, bending, reaching, lifting, and turning. It requires restraining confused clients, repositioning patients, assisting with transfers from wheel chairs/stretchers and transporting them to other departments or facilities. Mechanical devices are available and are required to be used for heavy or difficult to lift clients. The work may involve pushing or lifting in excess of 50 pounds and going up and down flights of stairs. The position requires visual and auditory acuity at a level to provide safe and effective nursing care. The position requires the nurse to work for periods of time with arms above shoulder level or with neck in a fully flexed or extended position and the ability to physically control or defend against emotionally ill clients. The position also requires ability to work on computers for extended periods of time.

Additional information

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
