    The Registered Nurse (Home Telehealth Care Coordinator) position is located at the Southeast Louisiana Veterans Health Care System New Orleans, LA in the Ambulatory Primary Care service.

    Responsibilities



    Registered Nurse (Home Telehealth Care Coordinator functions as a primary nurse for a defined group of patients. Responsible and accountable for all elements of the nursing process when providing direct and indirect patient care.

    • Considers all characteristics of the individual. Including age and life stages, state of health, culture, values and previous experiences. Core elements of performance include knowledge and active participation in program level quality improvement processes and initiatives and customer service programs.
    • Applies care and case management principles to coordinate care using health informatics, disease management, and home telehealth technologies to facilitate access to care and to improve the health of Veterans with the specific intent of providing the right care, in the right place, at the right time.
    • Promotes the goal of home telehealth to improve clinical outcomes and access to care while reducing complications, hospitalizations, and clinic or emergency room visits for Veterans in post-acute care settings and high-risk patients with chronic medical and mental heath disease.
    • All other duties for this position.

    Work Schedule: Monday -Sunday 7:30 am -4:00 pm as patient care dictates
