The Cincinnati, OH Department of Veterans' Affairs Medical Center is currently recruiting for a Registered Nurse (RN) to fill a RN, Case Manager position in the Mental Health Intensive Case Management (MHICM) Program, Assessment & Intensive Treatment Division section of the hospital's Mental Health Care line. This RN provides psychiatric nursing care to a wide variety of clients with DSM-5 diagnoses, including many patients with dual-diagnoses including substance abuse, as well as clients with complex psycho-social and behavioral issues. In addition, Mental Health RN's may provide 1:1 patient counseling and Community Case Management. This RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care and or Case Management. This RN conducts Bio-psychosocial assessments, directs medication management, performs any necessary procedures, facilitate orders for laboratory tests and procedures per established policies and guidelines and per Mental Health Intensive Case Management Program Policy. The Staff RN Case Manager is accountable that his/her patients receive the highest level of care. The Staff RN Case Manager is accountable and responsible to manage and protect VA resources. As part of your expected duties, will need to maintain an active unrestricted driver's license which will allow you when necessary to transport patients in the course of providing needed case management services. This transportation will be provided in approved government vehicles.
Other responsibilities include:
Participates in the national data collection for the MHICM Program, by completing required NEPEC data forms, per program policy.
Initiates treatment plans for assigned patients on case load, at time of admission and at 6 and 12 month intervals, and when there is a change in level of care for the patient.
Participates in MHICM treatment plan meetings and MHICM Quality Improvement Program.
Acts as a preceptor for orientees and to nursing students in advanced specialty training.
Tracks number of Case Management visits insuring optimal utilization of resources.
The staff nurse working in the Mental Health Care Line is accountable to the Chief Nurse Mental Health Care Line or his designee. The work schedule for this position will be primarily day tour, Monday Friday, however must be able to change schedule to accommodate the needs of the service.

Physical Requirements: The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. The VA Medical Center is a major interdisciplinary teaching facility, serving medical, surgical, and neurological inpatients and outpatients. There is a large outpatient psychiatric population, with admissions for medical co-morbidities. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, climbing stairs, and pushing. Transferring patients and objects over 40 pounds may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must be able to complete all levels of the Preventive and Management of Disruptive Behavior (PMDB) training as appropriate to the role. Fully participate in violence prevention program.

The Commission on Collegiate Nursing Education (CCNE) or
Foreign Schools of Professional Nursing, please note: In cases of graduates of foreign schools of professional nursing, possession of current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing or
MSN Bridge Program, please note: The completion of coursework equivalent to a nursing degree in MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Physical Standards. Per VA Directive and Handbook 5019 - a physical examination will be conducted based on the physical requirements of the position.
