The Community Care RN serves as a facilitator, for patient care, for Veterans seeking healthcare within and outside of the VA system, under the Veteran Choice Act. The incumbent will provide care coordination to link patients to VA and Non-VA healthcare services. The Community Care RN facilitates patient follow-up through patient coaching, information transfer, and communication in order to improve access, continuity of care, enhance the medical management of complex disease, and streamline the intervention process. When Non-VA Care is required, the Community Care RN will: obtain the required documentation for consult closure/management, coordinate, with Non-VA Providers, to meet post-appointment/discharge needs, including prosthetics (DME, home oxygen), home health, etc. The Community Care RN provides professional assessment, coordination and planning of multiple health care services: acts on behalf of the Veteran to assure that necessary clinical services are received, and monitor progress. In addition, the Community Care RN provides ongoing evaluation of care management services.
Key Responsibilities:
- Facilitate/coordinate care of patients both within and outside of the VA Health care system
-Maintain the Veterans Choice List, in appointment management in VistA
- Build and maintain therapeutic relationships with internal and external customers
- Provide data and maintain a tracking log/spreadsheet of all Veteran's Choice Veterans.
- Review medical documentation from VA and Non VA providers to determine provisional diagnosis and/or proposed treatment plans from VA and Non VA providers
- Provide ongoing education on the Veterans Choice Act
- Working effectively with patients, families/significant others, physicians, nurses and other healthcare professionals to optimize patient care.
- Working with case managers/social workers/transfer coordinators to facilitate post appointment follow up/discharge planning.
- Making decisions and carrying out actions with minimal supervision
- Learning other duties, under the Patient Flow Section of BMS, to provide coverage, when needed in areas such as Bed Control, NVCC, and Clinical Review.
Preferred/Enhanced Qualifications:
- Bachelor of Science in Nursing (BSN) Degree preferred
- A minimum of 3 years of progressive and successful medical/surgical nursing practice.
- Experience in non-VA care, utilization review/management, inpatient and outpatient care, and case management.
- Excellent interpersonal and customer Service Skills.
- Ability to work effectively in an Interdisciplinary Team.
- Ability to lead, prioritize, delegate, manage time, and meet deadlines.

Work Schedule: One RN will be expected to work Monday through Friday 0730AM to 400PM. The other RN will be expected to work Monday through Friday 0800AM to 430PM with occasional Bed Rotation coverage: 0600AM to 230PM; and 0930AM to 600PM.

Physical Requirements: Satisfactory physical examination as determined by the Employee Health Unit upon appointment only. Functional requirements include light lifting under 15 lbs, light carrying 15-44 lbs, walking and standing and standing for up to 8 hours, use of rapid mental and muscular coordination simultaneously, ability to read type written characters (glasses permitted), ability to hear the whispered voice (hearing aid permitted), repeated bending for up to 8 hours, etc.

Basic Requirements:

- Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
- The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
- English Language Proficiency: RN's appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).

- Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
