Major duties of the position include but are not limited to:

PCMM COORDINATION 65%

PCMM is a complex, nationally implemented, stand-alone computer program that is designed to monitor and manage primary care services throughout the VA system.

  • Monitoring and maintaining provider profiles, monitoring panel sizes, oversight and correction of transmission errors, and review/recertification of incorrect patient assignments.
  • Develops a process to track patients from enrollment and eligibility through vesting, New Enrollee Appointment Request (NEAR) report, and assignment to a PACT. Has primary responsibility for establishing and managing PCMM team and position set up for all primary care services at Medical Center.
  • Electronically create or modify team and position settings, assign provider user class based on verified credentials, link the associated clinic, set maximum panel sizes, program clinical alert notification levels, and establish preceptor to associate provider links. The incumbent uses multiple assignment/reassignment tools to move large groups of primary care patients to different teams and positions, when changes become necessary.
  • Notify patients via postal mail of their new team assignments and introduces the members of that team.
  • Works directly with the Nurse Managers and the Patient Advocate to track and complete clinic-wide patient provider change requests for patient satisfaction,
  • Works directly with a Pharmacy representative notifying them of all relocated patients that no longer require pharmaceutical support.
  • Responsible for notifying nationwide other PCMM Coordinators of patients transferring care outside of their VISN jurisdiction to allow proper panel management and capacity ensuring only qualified veterans are dual assigned across VISNs with proper CPRS documentation.
  • Responsible for verifying accuracy and correction of national duplicate assignments on a monthly basis.
  • Serves as the main point of contact for all new consults/referrals submitted to Primary Care directing them to the proper provider for care and assignment based on panel availability.
  • Monitors timeliness statistics for scheduling new patients, reporting to Primary Care leadership when problems exist which prevent compliance with VISN and VACO goals.
  • Accountable for initial and reinforced training for any Mental Health PCMM affiliates covering all aspects of the PCMM software and proper maintenance and legitimacy of inputted information.
  • Participates in patient education training as is related to outpatient systems organization, clinic access, scheduling, and logistical issues.
REPORTS 25%
  • Prepares national, VISN and Medical Center level reports, upon request using established and ad hoc computer programming routines.
  • Collects, compiles, analyzes and presents data regarding the integrity of the PCMM package and uses this analysis to prepare and develop PCMM program evaluation reports.
TRAINING AND RESOURCE UTILIZATION 10%
  • Responsible to notify and train all Primary Care leadership as well as the associated CBOC Managers for proper FTEE formulation for all employees which directly effects staffing and room utilization at the VISN level.
  • Prepares national, VISN, and Medical Center-level reports, using established and ad hoc computer programming routines.
  • Collect, compile, analyze, and present data regarding the integrity of the PCMM package, acting as a resource to clinical staff in troubleshooting of VistA and PCMM software applications associated with the Primary Care clinic treatment area.
  • Responsible for creating templates for report generation and updates/generates reports on a daily, monthly, or quarterly basis as requested or assigned.

Work Schedule: Monday- Friday; 07:00am-03:30pm. Schedule may be subject to change based on the needs of the Agency.
Telework: This position may be authorized for some ad-hoc (short term or episodic) telework only per Agency policy.

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 10/08/2026.

Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-09 position you must have served 52 weeks at the GS-07. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.

You may qualify based on your experience and/or education as described below:

  • Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to:
    • Knowledge in providing healthcare analytic project improvements for the service;
    • Knowledge of patient aligned care teams or similar groups in the healthcare environment and ensuring there adequate staffing;
    • Ability to gathering data and statistics for assigning patients;
    • Knowledge in planning, monitoring and developing new and existing programs, policies, and their administrative functions;
    • Knowledge in ensuring that administrative processes are initiated, monitored, and completed timely and accurately in compliance with applicable directives, policies, and procedures.;
    • Knowledge of various specialized databases, cloud based contracted software and systems to record, report, monitor and carry out administrative support operations;
    • Knowledge of preparing correspondence, i.e., spreadsheets, reports, emails, letters, etc. OR,
  • Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have 2 (two) years of progressively higher level graduate education leading to a masters degree or masters or equivalent graduate degree. Education at the graduate level in an accredited college or university meets the requirements for this positions at the GS-9 grade level. Such education must demonstrate the knowledge, skills, and abilities necessary to do the work . OR,
  • Combination: Applicants may also combine education and experience to qualify at this level. You must have a combination of specialized experience and education beyond one (1) year of graduate education. To calculate, first identify the percentage of required education you have earned (when substituting, you cannot use the first year of graduate education). Then identify the percentage of required experience you possess. Add the two percentages. The total percentage must equal at least 100 percent to qualify.
Physical Requirements: The work is sedentary in nature but requires walking, standing, and carrying medium weight items, such as light equipment/supplies, paper, books and charts. Manual dexterity is required in operation of a hospital computer system. Incumbent must have an even temperament and act in a professional manner during work performance.
