This Social Worker - Patient Aligned Care (PACT) Rural Native Veteran Healthcare Navigator is a professional social worker assigned to the VA Montana Health Care | Veterans Affairs Primary Care Clinics and Home and Community Based Services. The incumbent must possess skill in assessing and treating the complicated psychosocial problems of Veterans.
Duties include but are not limited to:
Establish and maintain effective therapeutic relationships with Veterans and/or their families. The incumbent will independently work with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems.
Independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families.
Actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions.
Maintain knowledge of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, common medications and their effects/side effects, and medical terminology.
Facilitate action for community placements through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure that appropriate community placements are completed in a timely manner.
Participate in development, implementation, and monitoring of contracts with Nursing Homes, including site visits and review of medical records.
Serve as a liaison between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services.
Support other programs in community outreach and home visits when deemed appropriate for service delivery or safety concerns (e.g. Care Giver Support Program).
Independently implement treatment modalities, provide educational classes, and/or supportive groups for Veterans and families.
Provide consultation and education to Veterans and their families regarding community resources, VA benefits and specialty programs, and advance directives.
Provide consultation to other treatment team and staff members regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veterans health care planning and compliance with treatment.
Maintain knowledge of Veterans benefits and services, community resources, and process for making appropriate referrals to community and other governmental programs or agencies.
Provide case management services to Veterans and their families throughout the continuum of care. The incumbent will be able to appropriately utilize principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness.
Communicate effectively, both orally and in writing with people from varied backgrounds.
As a Rural Native Veteran Healthcare Navigator (RNVHN) incumbent will assist care coordination and service delivery for individual PACT team enrolled Rural Native Veterans. Rural Native Veteran Healthcare Navigator (RNVHN) will support veterans navigating VHA as well as the Office of Tribal Health (OTH), Office of Tribal Government Relations.

BASIC REQUIREMENTS Citizenship.

Education.
Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure.
Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.

Education.
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one (1) year of professional social work experience in a clinical setting.

Preferred Experience: Case Management Medical Care Coordination; Social work preferred experience in assessment and treatment of complicated psychosocial problems of Veterans. Social Work experience in assessment and treatment of psychosocial problems often at a time when the Veteran is experiencing crisis, including the provision of short term, solution focused counseling with Veterans and/or their families. Experience working with individuals of native veteran population, homelessness, hospice services, nursing home care, home health services, financial resources, VA benefits, and transportation services.
The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-9 to GS-11.
Physical Requirements: Montana VA Healthcare System Primary Care utilizes the Patient Aligned Care Team (PACT) model wherein the incumbent works alongside all primary care team members in an open teamwork area designed to enhance direct access. Clinical appointments are conducted in private consultation rooms, while documentation and other administrative duties are completed from within the teamwork milieu. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in insolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situation, able to shift priorities based on patient needs.
