The Pharmacy Technician-Inpatient Hazardous Compounding (IHC) is a highly specialized tech performing a variety of complex functions in the preparation of chemotherapeutic agents. Most duties are performed in the oncology IV sterile compounding suite of pharmacy. This individual works under the Inpatient Pharmacy Technician Supervisor. The incumbent consistently demonstrates independent technical judgment and comprehensive, thorough working knowledge of sterile preparation for both hazardous and non-hazardous medications. Duties include:
Performing an essential role in the preparation, compounding, dispensing, and distribution of sterile and nonsterile hazardous medication within designated sterile compounding areas.
Receiving, reviewing, and preparing physician orders for cytotoxic agents and pre-medication.
Performing accurate product identification, labeling, dosage calculation, and compounding of agents utilizing package literature, written procedural guidelines, & specialized knowledge.
Successfully completing assignments & maintaining annual competencies to compound hazardous and non-hazardous IV sterile preparations according to USP <795>, <797>, & <800> standards.
Following proper aseptic preparation techniques when compounding hazardous & non-hazardous medications.
Reviewing anticipated inventory needs and notifies the procurement technician in a timely manner of products required.
Assisting in maintaining all required records related to sterile compounding.
Receiving hazardous IV chemotherapy inventory stock according to NIOSH guidelines, placing items in proper storage areas, rotating stock, & removing/replacing outdated items.
Maintaining Chemo/IV room medication inventory, compounding supplies, & cleaning products to ensure availability and minimize waste.
Conducting daily, weekly, & monthly IV room cleaning, including checking for soon-to-expire medications.
Manufacturing uncomplicated and/or complicated parenteral admixtures delivered as large volume, small volume, infusion syringe, or other forms as required.
Preparing sterile ophthalmic preparations & other complex sterile pharmaceuticals such as total parenteral alimentations & cardioplegia solutions.
Assisting in the central IV sterile suite as needed.
Applying comprehensive knowledge of USP 795/797/800 chapters & hazardous drug handling to assist with management, compliance, staff education, training, & room certifications.
Participating in glove fingertip & media fill testing, maintaining competencies by passing didactic & practical skill assessments initially & annually thereafter.
Assisting in airborne particulate, microbial, & surface testing to ensure cleaning schedules are met.
Ensuring full compliance with all regulatory requirements (VHA, Joint Commission, USP , EPA, OSHA, NIOSH, and FDA).
Providing training to new or existing staff to maintain required competencies.
Evaluating, analyzing, & coordinating workflow independently in the assigned area.
Performs all duties of lower-grade technicians, if required.
Assisting with inventory management & ordering of medications.
Monitoring refrigerator temperature daily in the pharmacy assigned area.
Working with, supporting, & substituting as needed for other Inpatient technicians, and staffs during staffing shortages.
Work Schedule: 7AM-3:30PM to include nights, weekends & holidays as needed Recruitment/Relocation Incentives (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Pharmacy Technician candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Certification: The employee must pass a national certification exam and hold an active national certification through either:
National Healthcareer Association (NHA), Certified Pharmacy Technician (ExCPT).
GS-8 Pharmacy Technician Experience: Candidates must possess one year of experience equivalent to the GS-7 grade level. Pharmacy technicians at the GS-7 level are nationally certified and can work autonomously using independent judgement to perform a full range of duties. These pharmacy technicians mentor and train lower graded pharmacy technicians in the practice setting. They assist advanced/ specialized pharmacy technicians and participate in performance improvement initiatives. They use independent judgement to perform the full range of duties in the pharmacy. They also function as staff pharmacy technicians in various locations of the pharmacy, including pharmacy contact centers and other non-traditional pharmacy locations, with minimal oversight.
GS-8 Pharmacy Technician Knowledge, Skills and Abilities (KSAs): In addition to the experience above, the candidate must demonstrate the following KSAs:
Comprehensive knowledge of concepts, principles, methodology and policies in a specialized area or section of pharmacy (such as sterile compounding, controlled substances, automation, clinical pharmacy, contact center, etc.) Note: See Specialized Area.in the Definitions Section.
Skill in training and orienting new and existing pharmacy facility employees and students on proper policies and procedures.
Ability to evaluate, analyze and coordinate workflow and work activities within a specialized area or section of the pharmacy.
Assignments: Pharmacy technicians at the GS-8 level have a higher level of responsibility for specialized areas of pharmacy and are expected to independently carry out assigned tasks of considerable difficulty. Pharmacy technicians in this assignment specialize in areas such as sterile compounding, oncology, controlled substances, medication history review, clinical specialist (anticoagulation, pain, infectious diseases), clinical informatics, automation, immunizations and any other specialized area in the advancement of the profession. The pharmacy technician will serve as a subject matter expert and key resource in the day-to-day technician duties of the specialized area. The technician will be performing assignments such as compounding sterile products, filling controlled substance prescriptions while maintaining compliance with Drug Enforcement Agency (DEA) regulations, taking medication histories and counseling patients, managing automation, managing workflow and training and orienting staff in the specialized area.
Preferred Experience: Previous pharmacy technician experience, inpatient technician experience, experience with sterile and/or hazardous IV compounding, leadership experience.
Physical Requirements: The work requires regular and recurring physical exertion, involving standing or sitting for prolonged periods of time, typing, walking distances within the medical center, lifting/carrying (up to 40 pounds) of pharmaceutical supplies, and bending/stooping/stretching.
Safety/Work Environment: Work is performed primarily in the inpatient pharmacy and outpatient care areas, where there is a potential for exposure to communicable diseases. Such areas are adequately heated and ventilated, and safety precautions are required. In some cases, dealing with anti-neoplastic drugs may expose the incumbent to hazardous and toxic agents. The incumbent works with drugs/chemicals and must exercise precautions to avoid accidental contact with the skin, eyes, and lungs. Follows life safety management (fire protection) procedures. Follows all hazardous pharmaceutical handling and disposal policies. Reports safety hazardous materials/Material Safety Data Sheets (MSDS)/waste management issues through appropriate channels. Follows Emergency Preparedness plan. Follows security policies/procedures. Complies with federal, state, and local environmental and other requirements preventing pollution, minimizing waste, and conserving cultural and natural resources.
VA Green Environment Management System (GEMS): Follows Healthcare System environmental policies and procedures related to the VA GEMS Program, and the recycling, safe use, and disposal of chemical products and high-risk medications. Understands and complies with federal, state, and local environmental laws and other requirements, preventing pollution, minimizing waste, conserving cultural and natural resources, and continually improving environmental programs. Completes environmental aspects and impact analysis for their activities in collaboration with the GEMS Coordinator, and effectively implements all applicable operational strategies developed through GEMS to ensure regulatory compliance.
