The incumbent serves as an Advanced Medical Support Assistant assigned to a Patient Aligned Care Team (PACT) model, in Primary Care, Mental health, Specialty Care Setting, Inpatient Ward, and Eligibility and Enrollments. The PACT team let is a primary care team that generally consists of a primary care provider (MD, NP, PA), registered nurse care manager, clinical associate (LPN or medical assistant/health technician) and an administrative associate
(MNMSA/health technician).

Major duties include but are not limited to:

  • Monitor and adjust clinic setup using utilization reports
  • Maintain effective communication with patients, interdisciplinary teams, VA facilities, and external agencies
  • Manage follow-up tracking systems for consults, tests, and care coordination
  • Participate in team huddles and meetings to plan and manage patient care
  • Prioritize work, adjust workflow, and ensure pre-appointment requirements are met
  • Manage electronic wait lists and resolve scheduling issues
  • Provide input on operational problems and perform administrative follow-up
  • Support continuity of care by sharing information across clinical teams
  • Verify patient eligibility, demographics, insurance, and means test data
  • Coordinate accurate scheduling, cancellations, rescheduling, and consult management
  • Facilitate inpatient-to-outpatient transitions, transfers, and patient travel activities
  • Ensure admissions, transfers, and discharges are processed correctly
  • Provide clerical support such as preparing letters, maintaining rosters, and running reports
  • Use CPRS, VistA, VistA Scheduling, VCM, and other systems to complete daily tasks

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: High school diploma, General Education Development equivalency certificate, or proficiency certificate from a State or territorial-level Board or Department of Education.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations:
GS-06: In addition to the Basic Requirements, candidates' resumes must reflect one year of experience equivalent to the next lower grade level, GS-05, which includes: Performing the full-range administrative support in inpatient and outpatient clinical settings; managing patient scheduling activities (appointments, consults, rescheduling, cancellations, no-show documentation); interpreting and verifying provider orders; monitoring and coordinating appointment requests from multiple electronic systems; supporting clinic operations through participation in huddles and daily workflow coordination; verifying and update patient demographic and insurance information; providing front-line patient support (phone coverage, greeting, message routing); coordinating administrative processes for patient transfers to between facilities; determining appointment types based on patient eligibility status.

The South Texas Veterans Health Care System (STVHCS) is comprised of two inpatient campuses: the Audie L. Murphy Memorial VA Hospital in San Antonio and the Kerrville VA Hospital in Kerrville, Texas. The STVHCS serves one of the largest primary service areas in the nation, with 35 counties, and is part of the VA Heart of Texas Veterans Integrated Service Network (VISN 17), with offices located in Arlington, Texas. Providing a service to Veterans with a working staff of more than 5900 employees, South Texas provides health care services for 114,000 Veterans.

The STVHCS is a facility providing primary, secondary and tertiary health care in medicine, surgery, psychiatry, and rehabilitation medicine. It also supports a 90 bed Extended Care Therapy Center, a 30-bed Spinal Cord Injury Center, and a Geriatric Research, Education and Clinical Center. Affiliated with UT Health San Antonio, the system has an active ambulatory care program with numerous satellite outpatient clinics.
