JOB TITLE: Social Worker Associate BUINESS TITLE : Substance Abuse Program Case Manager FLSA : Exempt JOB SERIES: 0187 GRADE: NF-04
NTRODUCTION: This position is located in the Behavioral Health Substance Abuse Program, Marine & Family Programs Branch, MCCS, MCB Camp S. D. Butler. The purpose of this position is to support the case management of the Behavioral Health SAP clinical and supervisory staff.
DUTIES AND RESPONSIBILITIES: The incumbent provides comprehensive case management services in support of ensuring clients receive timely, efficient, and effective clinical services in the areas of substance abuse and mental health. Plans programs of assistance and service for individuals and families he/she Works with, makes continuing appraisals of progress of plans, give sustained effective help and encouragement, and evaluate changes in the individual's or family's situation that may affect recovery program. Keeps all parties with "a need to know" informed of progress with respect to the various clinical services provided by the Substance Abuse Program (SAP).
Provides case management services for the SAP to ensure general mental health and counseling services in the continuum of care are provided in a timely, efficient, and effective manner. Schedules client appointments and monitors to ensure attendance, both with clinical provider appointments, and various other counseling modalities (e.g., group therapy and education classes). Ensures follow-up services for clients are provided. Manages the submission and follow up of referrals to community partners to include United States Naval Hospital (USNH) Substance Abuse Rehabilitation Program (SARP), USNH Mental Health Department, Combat Stress Team, Operational Stress Control and Readiness (OSCAR), MCCS Behavioral Health Community Counseling, and Family Advocacy Programs. Maintains up- to-date, accurate and complete records for all clients. Prepares agenda for the Interdisciplinary Team Meeting (IDT), attends such meetings, and all duties associated with the IDT meeting. Maintains up to date, accurate and complete records for all clients as well as monitors for quality assurance with respect to agency service delivery and record management. Inputs data in the Department of Defense Case Management System (DoD CMS), local trackers, maintains confidentiality and follows instructions, policy, regulations and Standard Operating Procedures on protecting client information. Monitors for quality assurance with respect to agency service delivery and records management.
Briefs and updates clients and Commands regarding case status, intervention and/or counseling recommendations resulting from the Inter-Disciplinary Team (IDT). Completing associated recommendation letters to commands. Communicates regularly with Substance Abuse Control Officers regarding client case status and IDT recommendations to the command. Promotes and maintains communication and liaison between clients, clinical staff, administrative staff, Command personnel and community agencies. Serves as command liaison in transferring or receiving active or closed SAP cases between commands due to Permanent Change of Station orders. Monitors deployment schedules to ensure unit members and commands have current SAC case status information.
Additionally, the individual must possess skills in areas of organizational management, time management and administration. Experience in the field of substance abuse is preferred, as well as knowledge of problem solving and prioritization of objectives, knowledge of military organizations, missions and lifestyles to include knowledge of federal, state and local human services policies, programs, procedures, organization and functions.
The ability to express orally and in writing in a highly professional and tactful manner, to meet and deal with diversified groups, individuals, organizations, and to compose written reports and material clearly, concisely and effectively.
Knowledge of legal information in the field of substance abuse and mental health as well as knowledge of the professional ethics of the substance abuse profession and ability to adhere to strict rules of privacy and confidentiality related to the management of these sensitive cases and related information.
BASIC REQUIREMETNS: 1. Successful completion of a 4-year course of study in human services or related field leading to a bachelor¿s degree from an accredited college or university and four years of experience in human services, social sciences, or related field. OR a Master¿s degree in human services or related field from an accredited college or university and two years of experience in human services, social sciences, or related field, or one year of specialized experience in case manager.
QUALIFICATION REQUIREMENTS: 1. Knowledge of the types of military, community, state and government organizations that provide service programs, which includes familiarity of local, state and federal regulations as they pertain to clinical services. 2. Knowledge of the most recent and current theories of substance misuse issues. 3. Knowledge in counseling services, family and group counseling, especially as they pertain to dysfunctional family dynamics with addicted members. 4. Knowledge and understanding of aspects of military life. Ability to communicate effectively, orally and in writing. 5. Knowledge of the military application of orders, instructions, and standard operating procedures is desirable. 6. A valid SOFA driver¿s license is required
