Help

Major duties of this position include but are not limited to...

  • Provides guidance and recommendations to primary care service line, working in collaboration with the Group Practice Manager, to review and ensure accuracy of data entered in to the Patient Centered Management Module (PCMM)
  • Makes recommendations to managers regarding allocation of resources, including potential uses of community care options
  • Proposes and leads process improvement activities to support implementation and improvement of PCMM review, assignments, and education in collaboration with Primary Care clinical and administrative leadership
  • Analyzes and interprets complex data. Researches and prepares comprehensive reports
  • Collects, analyzes, and trends data sets. Makes recommendations to assist in the allocation of resources
  • Develop tracking and reporting mechanisms to monitor process improvement and project management activities
  • Review regulations and policies and provide input at the local level for interpretation and implementation, utilizing clinic practice management and communication strategies
  • Attains and sustains knowledge of PCMM business rules and training materials to support continuous improvement processes
  • Primary manager of PCMM Web data base. Ensures that team assignments are set up correctly, routinely monitors data, and modifies to support data integrity for PCMM, MHMM, and HBPCMM
  • Acts as a subject matter expert for PCMM for the facility
  • Facilitates communication between Group Practice Management office, Clinic Profile Management Team, and clinical leadership/admin support teams to correct deficiencies, implement station improvement activities, and provides updated guidance on clinic practice
    management
  • Acts as a liaison between the Group Practice Manager and Service Line clinical and administrative leadership and serves as an active member of the Clinic Practice Management Work group/clinical operations team
Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.

Work Schedule: Monday - Friday, 8:00am - 4:30pm

Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/13/2026.

Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement.

For a GS-09 position you must have served 52 weeks at the GS-07 level.
For a GS-11 position you must have served 52 weeks at the GS-09 level.

The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.

You may qualify based on your experience and/or education as described below:

  • Specialized Experience GS-09: You must have one year of specialized experience equivalent to at least the next lower grade of GS-07 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to...knowledge and skill in applying analytical and evaluative methods and techniques to issues or studies concerning the efficiency and effectiveness of PACT, PCMM Web, Primary Care, and Mental Health clinic operations carried out by administrative or professional personnel, or substantive administrative support functions; knowledge of pertinent laws, regulations, policies and precedents which affect the PACT program, Primary Care, and Mental Health operations; knowledge to plan, schedule, and conduct projects and studies to evaluate and recommend ways to improve the effectiveness and efficiency of work operations in the PACT program, Primary Care, and Mental Health clinics; knowledge of the principles of process improvement and quality assurance in health care to implement, evaluate, and coordinate quality assurance and process improvement activities for PACT, PCMM Web, primary care, and mental health clinic operations; skilled in presenting recommendations and negotiating solutions to disputed recommendations; knowledge of PCMM, VistA, CPRS, VSSC; ability to utilize these systems effectively in the daily operations of PACT, primary care, and mental health clinic operations; and the ability to adapt quantitative analytical techniques to the measurement and improvement of the PCMM Web program effectiveness and/or organizational productivity...OR,
