Education: Successful completion of a graduate or higher level graduate degree that included 18 semester hours in the field of audiology with approved clinical practice. The education program must have been approved, recognized, or accredited by the Council on Academic Accreditation in Audiology and Speech-Language Pathology (CAA) of the American Speech-Language-Hearing Association (ASHA) at the time the degree was obtained. (Note: You must attach a copy of your transcripts with your application package).
License: A full, current, and unrestricted license to practice Audiology in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. (You must provide a copy of your license as part of your application package).

Experience: One year of experience which includes performing audio diagnostic/screening assessments; interpreting diagnostic tests; developing treatment plans for patients; and counseling patients and their families on plan of care.
