The incumbent in this position functions as HUD V ASH Program Coordinator for the Pacific Island Health Care System's HUD/VA Supportive Housing (HUD-VASH) Program. The HUD-VASH Program is at several sites throughout the Pacific Islands including Oahu, Kauai, and Guam. The incumbent is responsible for providing case management and other services for veterans receiving housing through HUD Section 8 Housing Choice Vouchers.

Duties include but are not limited to:

  • The incumbent actively participates in the HUD-V ASH Program virtual conferences and attends training offered by the VA. The incumbent actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedures or policy changes based upon work experiences. The incumbent is responsible for following local policies for the HUD-VASH Program. This includes following policies and documentation requirements as required for CARF and JC accreditation.
  • Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and conferences, and participating in training programs of other disciplines as requested. The incumbent performs accountability functions: 1) documents all contacts with patients/families in the medical charts; 2) completes statistical and other reports in accordance with VA Medical Center and the Homeless Operations Management and Evaluation System (HOMES).
  • Works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program veterans.
  • Is responsible for providing care coordination for a caseload of veterans in the HUD-VASH program, assigned at a ratio of 1 case manager for approximately 25 to 35 vouchers.
  • Verifies the veteran's status, eligibility for VA medical care, non-VA community care, and eligibility for program participation; performs site visits to HUD-V ASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes; and reviews agreements between Section 8 landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-V ASH placements.
  • The position involves driving to sites in the community and a valid driver's license is required.
  • Provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services including VA benefits, as well as community-based social services and other non-­VA entitlement programs.
  • Assists veterans in maintaining appointments at VA medical centers and other community agencies for medical, psychiatric, substance abuse, vocational and other services consistent with veteran's goals associated with healthful independent living.
  • Handles conflict and problems in dealing with the consumer constructively and appropriately. Has working knowledge and skill in the use of word-processing, data management and other computer systems, especially those programs in use by VHA.
  • Assumes the role of team leader in the HUD-VASH Program team system.
  • Organize work for team members, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
  • As appropriate, supervises the practice of social work students or less experienced social workers.
  • Other duties as assigned.

Work Schedule: Full-Time, Monday - Friday, 8:00am - 4:30pm

Pay: Competitive salary and regular salary increases.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
  • English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations:

Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.


Preferred Experience: LCSW; Two (2) years experience working with homeless, Mental Health/SMI, Substance use disorder IND, and working with Veterans.
