The Clinical Pharmacy Supervisor position includes the following functions, but other assignments and projects will be assigned as needed.

CLINICAL

  • Full time responsibility for providing pharmaceutical care to optimize patient response and resource utilization for professional staff. The incumbent carries out the following duties and responsibilities:
  • Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable.
  • Functions independently under scope of practice as defined by the medical center. This includes but is not limited to the following: Initiating, modifying or discontinuing medication therapy, ordering labs and other tests to help determine efficacy or toxicity of medications, designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing best evidence or VHA and/or CAVHCS published guidelines.
  • Involved in reviewing patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed.
  • Able to apply knowledge of normal laboratory values in the evaluation of patient care and recognizes significant abnormalities.
  • Expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings.
  • Reviews and implements Pharmacotherapy and treatment guidelines with providers.
  • Documents clinical interventions accurately in CPRS in a timely and professional manner as appropriate.
  • Works collaboratively with clinic providers to provide pharmaceutical care to selected patients (e.g., anticoagulation, hypertension, hyperlipidemia, diabetes, esophageal reflux disease, and smoking cessation) as outlined by best evidence, VHA and CAVHCS Clinical practice Guidelines.
  • Serves as an authority on drug usage, interactions, over-dosages, and adherence to medical staff, nursing staff, technicians and students. Uses appropriate references to research drug information.
ADMINISTRATIVE / SUPERVISORY
  • Develops, establishes, and maintains written policies and procedures defining operational requirements.
  • Supervises the implementation of service level programs, including clinical, operational, educational, and research activities.
  • Provides first level supervision to pharmacists, trainees, and other staff assigned to clinical pharmacy program areas.
  • Develops performance standards that are realistic, challenging, and support the organizational mission, vision, and goals. Ensure the employees meet performance standards and competencies and identify areas of improvement as needed.
  • Interviews employment candidates, makes selection recommendations, conducts performance reviews, recommends appointment, special advances, within grade increases or disciplinary action to the Pharmacy Professional Standards Board, Chief, Pharmacy Service, or Associate Chief, Pharmacy Service as appropriate.
  • Promotes good labor management relations and supports Equal Opportunity program initiatives in daily activities.
  • Maintains skills necessary to care for the aged population regarding differences in response to therapeutic agents and ability to adhere to medication management systems.
  • Establishes work schedules and allocates staff as needed to provide adequate coverage.
EDUCATION / RESEARCH
  • Effectively instructs patients and family members in the appropriate use of medications and medical devices and documents comprehension. Able to adjust communication and tracking methods based on age and developmental considerations. Refers patients to educational programs provided at the medical center by multidisciplinary teams as applicable.
  • Participates in the training of pharmacists, pharmacy technicians, and other relevant staff, in all aspects of training such as conducting on-the-job training and providing performance feedback to the Supervisor.
  • Assists in new employee orientation and training, as assigned.
  • Precepts and evaluates pharmacy students, pharmacy residents, and other relevant trainees, which may include projects, rotations, presentations, etc, if applicable.
  • Serves as an effective preceptor, directing and supervising pharmacy residents and students as assigned.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
  • NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
  • Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
GS-13 Grade Determinations: Experience. In addition to the GS-13 requirements, must have 1 year of experience equivalent to the next lower grade level of the GS-12.

GS-12 Grade Determinations - Clinical Pharmacist (Full Performance Level)
Experience or Education. In addition to the basic requirements, candidates must meet one of the following:
1 year of experience equivalent to the next lower grade level
OR
Completion of an ACPE-accredited Pharm.D. program.

GS-12 Pharmacists assigned to this position must demonstrate the following Knowledge, Skills and Abilities (KSAs):
a. Knowledge of professional pharmacy practice.
b. Ability to communicate orally and in writing to both patients and health care staff.
c. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
d. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

GS-13 Pharmacy Supervisor. Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains
effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation
and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area.

AND

GS-13 Pharmacists assigned to this position must demonstrate the following Knowledge, Skills, and Abilities (KSA)s:
a. Ability to effectively supervise subordinate staff.
b. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
c. Expert understanding of regulatory and quality standards pertaining to pharmacy.
d. Skill in managing people or programs.

Preferred Experience: PGY - 1 residency, Clinical Pharmacy Service and managerial experience preferred.

The full performance level of this vacancy is GS-12. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-13.
