Summary

The Outpatient Discharge Pharmacist works as a bridge between inpatient pharmacy and outpatient pharmacy discharge services. The Discharge Pharmacist will serve as the pass/discharge process expert and be involved in all aspects of medication management and pharmacy services. The Discharge Pharmacist will work with the inpatient staff hospital staff with full support and cooperation.

Help

Duties to include but not limited to:

Clinical Care/Dispensing Service:
1. Demonstrates accurate & timely dispensing of pass/discharge medications through automated dispensing device.
2. Initiates/provides clinical pharmacy services to the providers and staff. Provides medication treatment management utilizing an advanced in-depth knowledge of current literature and contemporary practice of medication.
3. Assists with medication reconciliation at point of discharge from inpatient services and acute hospital care and assists the provider with medication management challenges (e.g. non-adherence, adverse drug event identification and reporting, chronic narcotic use, polypharmacy, drug/drug interactions, etc.) as needed.
4. Improves clinical care to veterans through interventions to reduce the frequency of drug related problems and adverse drug events.

Customer Satisfaction:
1. Communicates in a manner that supports the veteran service goals of the medical center.
2. Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous and respectful manner.
4. Provides the customer with consistent information according to established policies and procedures.
4. Handles conflict and problems in dealing with the customer constructively and appropriately.

Administrative/Technical:
1. Adverse Drug Reaction (ADR) reporting.
2. Works with other health care providers to develop programs for improvement of drug use and the discharge process.
3. Ensures assigned administrative projects and reports are submitted in a timely fashion.
4. Assists in assuring that the clinic program is compliant with all regulatory and accrediting body requirements, i.e. The Joint Commission, FDA, ASHP, etc.

Program Support:
1. Assists in developing and updating various policies and operational procedures that affect the pass/discharge pharmacy time-out.
2. Incorporates performance improvement (PI) activities into all areas of practice.
3. Reviews non-formulary and restricted drug requests for appropriateness based on VHA Clinical Practice Guidelines and Criteria for Use (CFU).
4. Serve as liaison between the nursing staff, providers, and facility leadership/PBM to meet medication cost saving initiatives and assist providers and pharmacy service line to achieve targets set.

Education
1. Provides in-service education to the pharmacy staff, nurses, and other health care providers annually and as needed.
2. Participates in all aspects of training pharmacy students and residents.
3. Educates medical staff regarding formulary management, approved clinical guidelines and cost-effective use of drugs.

Security:
1. Protects patient and organization information from unauthorized release or from loss, alteration or unauthorized deletion.
2. Follow applicable regulations and instructions regarding access to computerized files, release of access codes, etc., as set out in VHA policy and memoranda.
3. Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, Federal regulations, VA statutes and policy, and VHA policy.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEF iBT).
  • Licensure: Hold a full, current, and unrestricted license to practice pharmacy in a State, Territory, or Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
  • English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Experience: 1 year of experience equivalent to the next lower grade level
OR
Completion of an ACPE-accredited Pharm.D. program

Grade Determinations: To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice.
  • Knowledge of professional pharmacy practice.
  • Ability to communicate orally and in writing to both patients and health care staff.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  • Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Outpatient pharmacy, medication reconciliation and CPRS/VISTA/ScriptPro experience

Physical Requirements: The work requires regular and recurring physical exertion, involving standing or sitting for prolonged periods of time, typing, walking distances within the medical center, lifting/ carrying (up to 40 pounds) of pharmaceutical supplies, and ending/stooping/stretching. Maintains safe and orderly work areas. Reports any incident of self/patient. Fills out appropriate form. Uses equipment and supplies appropriately.
