Distributive Functions

  • Verifies computer prescriptions (with provider order entry) for patients as assigned.
  • Directs pharmacy technician support, where applicable.
  • Coordinates medication orders with central pharmacy or CMOP when necessary.
  • For non-emergent medications, prescriptions will be entered for mail-out from CMOP (90-daysupplies will be encouraged for maintenance medications).
  • Participates with the pharmacy management in formulation and implementation of policies and procedures to assure accurate preparation, compounding and dispensing of all therapeutic and diagnostic agents and other supplies furnished by Pharmacy Service to beneficiaries through the various inpatient and outpatient functional areas.
  • Performs other clinical and staff pharmacist duties as required in periods of acute staffing needs.
  • Adheres to pharmacy policies and procedures regarding prescription entry and dispensing of outpatient-controlled substances
Clinical Functions:
  • Provides clinical coverage in the pharmacist-run clinics in pain management (or other areas as assigned) as well as supportive clinical care where appropriate through asynchronous means.
  • Provides the full range of resources as a specialist in drug information covering a wide range of therapeutic classes and especially in the area of his/her specialty training. Advises other members of the medical/surgical team as well as other health care providers on all matters relating to medication, contraindications, drug interactions, formulary medications.
  • Reviews the patient's medication profile for any possible drug allergies, incompatibilities, interactions, duplications, or compliance problems.
  • Serves as the pharmacy representative on the interdisciplinary teams when assigned. Responsibilities include, but are not limited to reviewing patient medication profiles, monitoring outcomes and/or recommending alternative therapy. Must have appropriate knowledge of disease states and clinical pharmacology.
  • Actively participates in the Medical Center's medication use reviews/studies to assess our medication use patterns including treatment of pain. Also participates in evaluating new drug requests and non-formulary requests for the inpatient pharmacy.
  • When assessing or providing patient care the employee will evaluate the impact of the age and disease status and factors as it relates to the patient's ability to understand directions provides objective self-assessment or any other medical milieu appropriate to the overall treatment plan.
Educational Functions:
  • Precepts pharmacy students, PGY-1 and PGY-2 pharmacy residents including providing constructive feedback regarding quality of educational experience and preceptor interaction/involvement.
  • Participates in the education and training of new employees or employees introduced to selected practice areas.
  • Participates in the ongoing education, business activities and policy review of medical center staff as it relates to the provision of drug therapy and drug information as necessary.
  • Provides patient education through direct consultation, handouts, and special medication groups.
  • Maintains record of contribution to pharmacy practice as defined by ASHP for preceptors and completes assigned residency assignments in a timely manner.
Program Management & Administrative Functions:
  • Developing processes/procedures to ensure implementation and compliance with national policy related to pain management, opioid safety and risk mitigation strategies, Overdose Education and Naloxone Distribution (OEND), management of Opioid Use Disorder (OUD), initiatives in compliance with national policy, clinical practice guidelines, and regulatory requirements.
  • Supporting review of community provider opioid prescribing practices as required by the MISSION Act Section 131.
  • Supporting OSI reviews, data-based risk reviews for opioid-exposed patients (including Stratification Tool for Opioid Risk Mitigation (STORM) data-based reviews), and other interdisciplinary pain care forums.
  • Supporting and tracking full implementation of the Stepped Care Model for Pain Management (SCM-PM), including a high functioning PMT/Pain Clinic at the facility, and access to tertiary pain center within the VISN including a CARF-accredited interdisciplinary pain rehabilitation program.
  • Facilitating implementation of the OUD stepped care model within the facility, in particular for Step 1 of the Stepped Care for Opioid Use Disorder Train-the-Trainer (SCOUTT) program expansion, within PMTs/Pain Clinics.

Clinical Pharmacists assigned to this position must demonstrate the following KSAs:

  • Knowledge of professional pharmacy practice.
  • Ability to communicate orally and in writing to both patients and health care staff.
  • Knowledge of laws, regulations, and accreditation standards related to the distribution and control of schedule and non-scheduled and non-scheduled drugs and pharmacy security.
  • Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D. degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy).
  • TRANSCRIPTS MUST BE SUBMITTED (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). YOU MUST SUBMIT A COPY OF FPGEC CERTIFICATION WITH THE APPLICATION PACKAGE
  • Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
  • YOU MUST SUBMIT A COPY OF CURRENT LICENSE WITH THE APPLICATION PACKAGE.
English Language Proficiency. Pharmacists must be proficient in spoken and written English in accordance with 38 U.S.C. 7402(d) and 7407(d).


Experience. In addition to the basic requirements, candidates must meet the following:
One year of experience equivalent to the next lower grade level (GS-12) which includes Knowledge of Professional Pharmacy Practices, Residency accredited by the American Society of Health-System Pharmacists (ASHP) and Fellowship Training Program accredited by the American College of Clinical Pharmacy (ACCP)


Preferred Experience: PGY2 in pain or palliative care OR experience providing direct patient care for pain as a CPP or in residency training
