Miami VA Healthcare System is looking for a Social Worker. This position is in the Health Care for Homeless Veterans (HCHV) program within the Social Work Service. They will serve as a primary point of contact with the partnering local Public Housing Authorities, provide comprehensive social work services to homeless Veterans and their families enrolled in the HUD-VASH program.

Total Rewards of a Allied Health Professional

The Social Worker will provide comprehensive social work services to homeless Veterans and their families enrolled in the HUD-VASH program, ensure the Veteran's care is appropriately coordinated with the VA Medical Facility, community health or mental health providers, and agencies that serve homeless populations. Position will require considerable travel into the community to meet with community agencies and partners who provide assistance and support to the HUD-VASH Program and its Veterans.

Additional Duties Include But Not Limited To:

  • Provide housing search, case management, and services for a caseload of Veterans in the Housing and Urban Development - Veterans Affairs Supportive Housing (HUD-VASH) program.
  • Responsible for reviewing agreements between Veteran participants and landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.
  • Perform site visits to rental properties, apartments, and houses to ensure they are safe environments and in compliance with local housing codes and housing quality standards (HQS).
  • Assist the HUD-VASH team by verifying the Veteran's status, eligibility for VA medical care, and eligibility for program participation.
  • Ensuring homeless Veterans who meet criteria for chronic homelessness are considered as primary participants for program acceptance.
  • Collect and submit HUD-VASH Program participant data.
  • Provide HUD-VASH clinical services.
  • Coordinate care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the VA Medical Center, VA Regional Offices, and community agencies.
  • Participate in HUD-VASH conference calls and attends training offered by the HUD-VASH Program.
  • Participate in program planning and development, advising the HUDVASH Supervisor of shifting trends with housing stock, housing agencies, caseloads, and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
  • Document all contacts with patients/families in the electronic medical record.
  • Collaborate with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering housing services and resources to HUD-VASH Program Veterans.
  • Participate in HUD-VASH discharge planning with other disciplines; responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.
  • Provide individual, family, and group counseling.
  • Provide consultation/education to Veterans and families on community housing resources, advance directives, and VA benefits.
  • Act as a consultant to other team/staff members and community providers on issues related to local housing resources.
  • Act as liaison between VA and landlords, property managers, Housing Authorities, and community housing resources, to include furniture banks, thrift shops, and other relevant resources.
  • Provide clinical case management to HUD-VASH Veterans and their families throughout the continuum of care.
  • Collaborate and network with other housing service providers, housing specialists, and housing locators to address issues of homelessness and housing for Veterans in the HUD-VASH program.
  • Provide housing resource education and facilitate patient/family housing support as appropriate. This may include housing resource support groups.

Work Schedule: Monday through Friday, 8:00 am to 4:30 pm, with rotating tours of duty as needed. Additional duties include weekend and holiday coverage within Social Work Service on a rotating basis.
Recruitment/Relocation Incentive (Sign-on Bonus): Not Authorized
Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f).
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.

GS-11 Social Workercandidates must have the following:
(1) Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non- VA experience) and licensure or certification in a state at the independent practice level.

OR

2) Education.
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting.

  • Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
  • Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
  • Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i .e acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
  • Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
  • Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.

Physical Requirements: The physical demands of the work require use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive a government car.

Receiving Service Credit for Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
