Duties

Help
  • Serves as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
  • Facilitate optimal patient outcomes by aligning treatment plans with best practices and the unique needs of each patient.
  • Providing individual and group support, psychoeducation, and skills training on topics such as stress management, coping mechanisms, and relapse prevention.
  • Design timeline-based care protocols that ensure high-quality, cost-effective healthcare for patients and beneficiaries under case management.
  • Develop, implement, communicate, and coordinate individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
  • Observe and evaluate patient behavior and responses, updating treatment plans in consultation with the healthcare team.
  • Chairs and coordinates the weekly At-Risk/Multi-Disciplinary Treatment Planning (ARCT/MDTP) meeting to ensure treatment plans for at-risk and high-interest patients are synchronized among all medical and support treatment providers.
  • Develop and recommend strategies to improve healthcare access, reduce administrative barriers for beneficiaries, and enhance the efficiency of the MTF.

  • This position requires that you obtain and maintain a Basic Life Support (BLS) certification from, or in accordance with, the American Heart Association and American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS.
  • This position requires the incumbent be able to obtain and maintain a determination of eligibility for a Secret security clearance or access for the duration of employment. A background investigation and credit check are required.

  • This position has mandatory seasonal Influenza vaccination requirements and is subject to annual seasonal Influenza vaccinations unless otherwise exempted for medical or religious reasons (documentation of exemption must be provided upon hire).
  • This position requires the incumbent to have Tuberculosis testing.
  • This position may require the incumbent to work on call during irregular work hours which may include evenings, weekends, holidays, and/or varying work schedules.

In order to qualify, you must meet the education and experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Basic Requirement for Behavioral Health Case Manager Degree:
Bachelor's or Graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of Behavioral Health Case Manager.

In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

Specialized Experience: One year of specialized experience equivalent to at least the GS-11 grade level in the Federal service which includes documented experience: 1) assisting patients with access to healthcare systems or resources; 2) collaborating with multidisciplinary healthcare teams to develop treatment planning for patients; 3) utilizing electronic health records or similar systems to document care coordination activities and/or track outcomes. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).

Education

Some federal jobs allow you to substitute your education for the required experience in order to qualify. For the GS-12, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.
