This Home-Based Primary Care (HBPC), Geriatrics and Extended Care Service, Program Director/Coordinator position is a Registered Dietitian; and is accountable for the management, supervision and evaluation of care for patients enrolled in the HBPC, which includes all facility and CBOC locations within CTVHCS.

Major Duties as Assigned, but not limited to:

  • Establish performance standards and evaluate the overall performance of employees; define competency requirements, identify developmental and training needs and take necessary actions to ensure subordinate personnel maintain and enhance technical expertise.
  • Formulate budget and staffing requirements in managing organizational changes and ensuring multi-year and long-range national 22 mission directives and program goals/objectives are met.
  • Provide program-specific guidance and direction and technical advice and staff development.
  • Oversight responsibilities for program and policy and procedure development, set goals and monitor and redefine priorities and may have an active role in fiscal management for the program.
  • Maintain program information through approved social media outlets.
  • Provides senior leadership and oversight for daily clinical and administrative operations.
  • Responsible for the development and continued effective functioning of the HBPC interdisciplinary health care team recognizing and utilizing the skills, knowledge, and contributions of each team member.
  • Facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members.
  • Responsible for the HBPC team development and implementation of local HBPC policies and procedures.
  • Responsible for the evaluation of the effectiveness of the program and assuring effective/efficient resources are maintained.
  • Collaborates with the HBPC Medical Director to manage human resources for the HBPC Program including selecting qualified staff, orienting and mentoring staff, promoting staff development and professional growth, and assuring a process for ongoing staff clinical competency, including the continuing education needs of program staff and the appropriate resources to meet those needs.
  • Supports the integration and collaboration of all professional disciplines and functional areas in the mutual achievement of patient-centered and organizational goals.
  • Collaborates with interdisciplinary team members, including other program and service leaders at the facility and network. They work closely to establish and maintain effective relationships with all levels of Veteran Affairs (VA) staff nationwide.
  • Assist the HBPC Medical Director with healthcare operations and the functions and activities expected of them.
  • Contributes ideas and recommendations for the establishment of standards of care, policies, and objectives for the enhancement of patient care.
  • Ensures assigned program follows statutory law, accrediting organizations, and Veterans Health Administration (VHA) regulations, policies, and procedures.
  • Responsible for directing the clinical and administrative services offered by the program to ensure that the program is in compliance with local and VHA standards and policies as well as accreditation standards for home care organizations.
  • Responsible for the interpretation and communication of VA policies to the HBPC team and the VHA facility.
  • Processes, assigns, and tracks referrals for HBPC.
  • Manages the Electronic Waiting List (EWL) for HBPC referrals
  • Provide assistance, collaboration, and subject matter expertise for a complex group of services HBPC often works with, such as Home Oxygen Programs, Home Medical Equipment and contracting services to comply with regulatory standards.
  • Responsible for evaluating program activities and providing follow up actions required by internal and external review organizations and accrediting bodies.
  • Responsible and accountable for directing clinical services in HBPC.

Qualifications

Basic Requirements:

  • Citizenship. Be a citizen of the United States (U.S.). Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with 38 U.S.C. § 7407(a).
  • Registration. All applicants must hold the RDN credential from the Commission on Dietetic Registration (CDR) the credentialing agency of the Academy of Nutrition and Dietetics. Note: Applicants who hold the RDN credential have met the education requirement (e.g. bachelor's degree or higher) of CDR, as such this occupation is considered professional and scientific with a positive education requirement and the procedures in Part I, Chapter 4 of this Handbook will be followed when applying Veteran's preference.
  • English Language Proficiency. Dietitian candidates must be proficient in spoken and written English in accordance with 38 U.S.C. § 7403(f).
  • Physical Requirements. See VA Directive and Handbook 5019, Employee Occupational Health Service for requirements.
Current VHA Dietitians that qualify based on being covered by the Grandfathering Provision may not be promoted to a higher grade if they do not meet the current RDN/RD requirements.

Grade Determinations:

GS-13

Experience. One year of experience equivalent to the next lower grade level.
Knowledge, Skills and Abilities. In addition to the experience above, the candidate must demonstrate the following KSAs:
  • Skill in developing quality improvement or research activities.
  • Skill in developing training and reference materials.
  • Ability to develop policies and procedures.
  • Ability to provide program-specific coordination to include leadership, technical advice and staff development.
  • Ability to administer a program including establishing and modifying program goals and objectives, coordinating activities, evaluating accomplishments and solving problems.
Preferred Experience:
  • 2 to 5 years of formal leadership experience.
  • 1 to 2 years of formal VA leadership experience.
  • 3 to 5 years of experience providing care primarily to Geriatric patients.
  • 1 to 2 years of experience in home care.
  • 1 to 2 years of supervisory experience in the VA.

The full performance level of this vacancy is GS-13.

Physical Requirements: The work is sedentary. Some work may require movement between offices, hospitals, warehouses, and similar areas for meetings and to conduct work. Work may also require walking/standing, in conjunctions with travel to and attendance at meetings and/or conferences away from the work site. Incumbent may carry and lift light items weighing less than 15 pounds.
