The Home Based Primary Care (HBPC) Assistant Program Director is a Dietitian (Facility Program Coordinator) who assists the HBPC Program Director to be accountable for the management, supervision and evaluation of care for patients enrolled in the HBPC programs that are organizationally aligned under HBPC, which includes all facility and CBOC locations. He/she is under the supervision of the HBPC Program Director under Geriatrics and Extended Care Service. The Assistant Program Director assumes full responsibility for the HBPC Program in absence of the HBPC Program Director. Some of the duties include, but are not limited to:

  • Works with the HBPC Program Director to direct the clinical and administrative services to ensure that the program is in compliance with local and VHA standards and policies as well as accreditation standards for home care organizations.
  • Assists with managing and tracking program resources to include but not limited to equipment inventory, GSA and VA owned Vehicles.
  • Tracks referrals for HBPC and ensures timely access to HBPC.
  • Manages the consult pending list for HBPC referrals.
  • Collaborates with the business office and fiscal service to ensure accurate reporting of HBPC clinical, administrative and workload data.
  • Collaborates with the HBPC Program Director and HBPC Medical Director to manage human resources for the HBPC Program, including selecting qualified staff, orients and mentors the staff, promotes staff development and professional growth, and assures a process for ongoing staff clinical competency.
  • Assesses the continuing education needs of program staff and provides on-going learning experiences that enhance the employee's knowledge and skills.
  • As a leadership member of the Geriatrics and Extended Care Service, he/she participates in the formulation of overall service/program policies and procedures, goals, objectives, strategic planning, cost effectiveness, and quality monitoring.
Work Schedule: The primary tour of duty will be M-F 8am to 4:30pm. However, the selectee may also be required to work various shifts on a permanent or temporary basis, based on the needs of the service and patient care.

Qualifications

Basic Requirements:

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

English Language: Must be proficient in spoken and written English

Registered Dietitian Nutritionist (RDN) or Registered Dietitian (RD) -Documentation Submitted: Must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA).

Current VHA Dietitians that qualify based on being covered by the Grandfathering Provision may not be promoted to a higher grade if they do not meet the current RDN/RD requirements.

Grade Determinations: All individuals assigned to this occupation must meet all the basic qualification requirements above and all the requirements defined in the specific assignment.

Dietitian (Facility Program Coordinator), GS-12.

Experience Requirement: In addition to meeting the basic requirements, to qualify for the GS-12 you must possess at least 1 year of experience as an RDN/RD equivalent to the next lower grade level (GS-11).


Knowledge, Skills and Abilities. In addition to the experience above, the candidate must demonstrate the following KSAs:

  1. Knowledge of program administration establishes program goals and objectives, coordinates activities, evaluates accomplishments and solves problems.
  2. Skill in developing quality improvement or research activities.
  3. Skill in developing training and reference materials.
  4. Ability to develop policies and procedures.
  5. Ability to provide program-specific coordination to include leadership, technical advice and staff development.

Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty) and variety and be performed by the incumbent at least 25% of the time. Employees in this assignment serve as Facility Dietitian Program Coordinator leading a multi-disciplinary team. Dietitian Program Coordinators may provide direct patient care by developing and implementing comprehensive nutrition therapy, practicing within their scope/clinical privileges. They are administratively responsible for a specialty program and provide program-specific leadership, guidance and direction, technical advice and staff development. Dietitian Program Coordinators oversee responsibilities for program, policy and procedure development, set goals, monitor and redefine priorities and may have an active role in the fiscal management for the program. They direct or participate in the design of research activities or quality improvement studies, including data collection and analysis. Dietitian Program Coordinators have oversight of and are responsible for developing and maintaining training and reference materials. They maintain program information through approved social media outlets. Dietitian Program Coordinators may supervise up to two employees. This assignment must represent substantial additional responsibility over and above that required at the full performance grade level and cannot be used as the full performance level of this occupation.

Physical Requirements: This position requires performance of work which involves light to moderate physical effort, such as that involved in lifting files, bending, standing, reaching, operating office equipment such as telephone, copier, fax, scanner, etc. The physical demands of the work requires use of fingers; moderating walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; hearing aid permitted; and clear speech. Must meet occupational health requirements for driving due to the requirement to drive a government owned vehicle for numerous hours on an intermittent basis. The position requires emotional and mental stability and often places the incumbent in a position of stress.
