Summary

This position functions as the Charles George VA Medical Center Liaison/Case Manager Homeless Providers Grant and Per Diem (GPO) Program. This position is responsible for monitoring and coordinating services provided by community-based programs funded under VA's Homeless Providers GPO Program. This position is also the key point of contact between these community-based providers and the National GPO Program Office, Tampa, FL.

Duties include but are not limited to:

  • Works in coordination with the HCHV Program Coordinator to provide a seamless system of healthcare services.
  • Provides case management at homeless community- based GPO sites as described in the GPO Program rules, regulations, and directives.
  • Utilizes various treatment modalities of individual and group work, casework, community organization, and community outreach activities to alleviate or remove problems that interfere with GPO Program participants' ability to achieve independence.
  • Monitors the clinical care and assesses the compliance of the programs receiving per diem as described in the grant recipient's original grant application.
  • Intervenes to facilitate compliance or corrective actions and, when appropriate, develops, facilitates, and monitors plans of correction when necessary.
  • Provides services and oversight of the GPO funded programs as defined in the GPO rules, regulations, and directives. This may include verification of veteran status and eligibility for participation in the GPO Program, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care, collecting and submitting participant information as outlined by NEPEC evaluations procedures, and tracking scheduling for and attendance at required VA Mental Health/ Substance Abuse, and/or medical appointments.
  • Coordinates and participates in annual program site inspections utilizing the GPO Program rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices.
  • Notes inspection deficiencies and establishes methods to monitor corrective actions when necessary. This process may include development of timelines for consequences as a result of inaction or insufficient action related to deficiencies.
  • Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies.
  • Engages in coordination with community agencies, organizations, and groups to learn of services or resources available for homeless veterans as well as provides current information on services and benefits available in the VA for Homeless GPO Program participants.
  • Promotes the Grant and Per Diem Program by providing education to community agencies, organizations, and groups on the Grant and Per Diem Application process to ensure a seamless continuity of care of homeless residential housing services. The incumbent complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the Homeless GPO Program Liaison/Case Manager.
  • Participates in the HCHV Program performance improvement measures including monitoring clients' compliance with attendance at mandatory medical, mental health/substance abuse appointments.
  • Actively participates in the GPO Program conference calls and attends training offered by the GPO Program.
  • Actively participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences.
  • Participates in the orientation/teaching of graduate social work/nursing students, students of other disciplines, and VAAAHS personnel on the biopsychosocial aspects of homelessness.
  • Takes responsibility for improving skills of other professionals, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.
  • Performs the following accountability functions: 1) documents all contact with patients/families in the medical charts; 2) completes statistical, administrative, staffing studies and other reports in accordance with VAAAHS and respective professional discipline service policies.
  • Works with Charles George HCHV multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to Homeless GPO Program veterans.
  • Arrange and coordinate veteran's care; link and referral veterans to the VA medical facilities, VARO, and intervene on behalf of the veterans to fill gaps in the delivery of services.
  • Shares site specific Northeast Program Evaluation Center (NEPEC) data with GPO community providers quarterly or as available.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ยง 7403(f).
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.
    • Loss of Licensure or Certification: Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment. Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

  • Social Worker (Program Coordinator), GS-12
    • Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
    • Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs:
    • (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
    • (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
    • (c) Ability to supervise multidisciplinary staff assigned to the program.
    • (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
    • (e) Ability to provide training, orientation, and guidance within clinical practice.
