Duties

Help
  • Serve as a behavioral health case manager on a multidisciplinary medical team, conducting complex assessments and coordinating care for high-risk patients.
  • Develop and implement individualized treatment plans, monitor medication effects, and provide psychoeducation, skills training, and therapeutic interventions.
  • Facilitate care transitions and collaborate with internal and external providers to ensure continuity and quality of care.
  • Lead weekly treatment planning meetings, analyze patient data to identify trends, and recommend program improvements.
  • Analyze patient data to uncover trends, guide program improvements, and support smooth transitions between levels of care.

  • Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
  • Employee must obtain and maintain a favorable result from a Criminal History Background Check (CHBC). This position requires at least one contact per week or at least four (4) contacts per month with children under the age of 18.
  • This position may be designated "Mission Essential." In the event of severe weather conditions or other such emergency type situations the incumbent is required to report to work or remain at work as scheduled to support mission operations.
  • This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination.
  • This position is a Testing Designated Position. All applicants tentatively selected for this position will be required to submit to urinalysis to screen for illegal drug use prior to appointment.
  • Must obtain and maintain a current Basic Life Support (BLS) training and certification from the American Heart Association or Military Training Network within 30 days of employment.
  • Employees assigned to a medical treatment facility are required to be screened for applicable immunizations or pre-existing immunity based on occupational risk, regardless of job series.
  • Hepatitis B immunization is required for all positions with direct patient contact.
  • May be required to work other than normal duty hours to support non-duty hour classes.
  • May be required to possess a valid operator’s license in order to drive a motor vehicle to and from work sites.

Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Basic Requirement for Behavioral Health Case Management:

Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

Specialized Experience: One year of specialized experience which includes conducting behavioral health assessments/interviews; coordinating behavioral health care plans with a multidisciplinary team; and evaluating appropriateness, quality, or cost effectiveness of care. This definition of specialized experience is typical of work performed at the second lower grade/level position in the federal service (GS-11).

Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.

Education
