Summary

The PACT Clinical Pharmacy Specialist Supervisor serves as a Clinical Pharmacy Specialist (CPS) pharmacist with knowledge and proficiency in pharmacy practice and pharmacy administration of the Patient Aligned Care Team (PACT) programs for all three (3) primary care locations within Salisbury VA Health Care System. The supervisor will be responsible for the personnel management of pharmacists supporting clinical pharmacy services in the PACT areas.

The duties of Supervisory Clinical Pharmacy Specialist (PACT) include, but are not limited to:

Administrative/Supervisory Duties:

(1) Oversees the direction and completion of work by subordinates to ensure timeliness, accuracy, and completeness. 2) Approves or disapproves leave requests, formulates work schedules, performs timekeeping functions, and ensures appropriate staffing for the work of the section. 3) Assesses workload trends and needs and adjusts staffing requirements as needed to maintain access to care. Performs CPS duties as needed based on daily staffing needs. 4) Develops and maintains systems to monitor the performance of staff activities. 5) Ensures performance standards are maintained, staff orientation needs are designed and implemented, and clinical care reviews are performed. 6) Maintains effective intra-department relations and inter-department relations to accomplish the facility missions and goals. 7) Develops effective communication with AFGE and is knowledgeable on the master and local agreement. 8) Assures security over pharmacy areas and provides an immediate report to management in event of inconsistencies. 9) Assures clinical services within pharmacy service comply with regulatory and accreditation body standards, best practices, and coincide with the overall mission of the Salisbury VA Health Care System.

Customer Service:

(1) Treats patients, team members, other facility employees and visitors with respect. 2) Responds to the needs of the Veteran and staff in a courteous and timely manner. 3) Demonstrates appropriate and proactive interpersonal skills to achieve the goals of the service. 4) Acts as a team member and always supports efforts of both the pharmacy and clinical pharmacy specialist team. 5) Demonstrates an ability to work under a variety of circumstances to maximize group productivity and quality.

Clinical - Direct Patient Care Activities that require a scope of practice:

(1) Core functions include face-to-face and virtual patient care (e.g., telephone visits, tele-health, and other virtual modalities) with PACT team patients, for comprehensive medication management in practice areas identified in their individual scope of practice. 2) Functions as a mid-level provider to design, implement, and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers in covering areas. 3) Orders, performs, reviews, and analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. 4) Follows-up with patients on lab or test results to discuss the plan of therapy, i.e., changes in medication therapy, monitoring, and additional testing requirements. 5) Clinical pharmacy specialists will determine if a referral to the physician is necessary when disease progression occurs, or adverse drug events require treatment interventions outside of the CPS scope of practice.

Clinical - Indirect Patient Care Activities:

(1) Identify patients who may benefit from comprehensive Medication management through population management reports and during team huddles 2) Review patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. 3) Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues. Documents findings per facility procedures.

Supervisory Controls:

The PACT CPS supervisor serves with considerable independence in all areas of pharmacy and supervisory activity under the supervision of the Associate Chief of Clinical Pharmacy programs. The pharmacist is also accountable to the Chief, Pharmacy Service, or their designee, for professional licensure, competency, and continuing education.

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: You must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
  • Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/.(NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
  • Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination(FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations:

Creditable Experience
(1) Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board.
(2) Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).
(3) Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination.

Grade Determinations: In addition to the basic requirements for employment in paragraph 2, the following criteria must be met when determining the grade of candidates.

GS-13 Pharmacy Supervisor. Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area. Pharmacists assigned to this position must demonstrate the following KSAs:

a. Ability to effectively supervise subordinate staff.
b. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
c. Expert understanding of regulatory and quality standards pertaining to pharmacy.
d. Skill in managing people or programs.

Preferred Experience: Candidates for the PACT Clinical Pharmacist Practitioner Supervisor position should possess the skills with specific experience in Ambulatory or Primary Care setting. Candidates should have received experience in a current or previous job (including residency training) that includes direct patient care and comprehensive medication management responsibilities. Preferred candidates will have combination of the following: PGY1 Residency, PGY2 specialty residency, and/or board certifications (BPS); or equivalent training and experience providing direct patient care as a pharmacist provider.

The full performance level of this vacancy is GS-13.

Physical Requirements: The work is sedentary. There may be some walking, standing, bending, and carrying of light items.
