Help

Serve as a behavioral health case manager on a multidisciplinary medical team, conducting complex assessments and coordinating care for high-risk patients.
Develop and implement individualized treatment plans, monitor medication effects, and provide psychoeducation, skills training, and therapeutic interventions.
Facilitate care transitions and collaborate with internal and external providers to ensure continuity and quality of care.
Lead weekly treatment planning meetings, analyze patient data to identify trends, and recommend program improvements.
Analyze patient data to uncover trends, guide program improvements, and support smooth transitions between levels of care.

This position is subject to a background investigation to determine your suitability for Federal employment.
This position has specific initial background investigation and periodic re-verification requirements since its duties require interaction with children and youth under the age of eighteen (18) years of age on a recurring basis.
May be required to work other than normal duty hours to support non-duty hour classes.
The duties of this position require the incumbent to possess or obtain and maintain a valid state Driver's License in one of the 50 U.S. states or possessions to operate vehicles.
This position requires the completion of a pre-employment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health and ability or fitness to perform the duties of the position.
Lifting up to 40 pounds may be required.
This position may require the incumbent to get a seasonal influenza vaccination and may be subject to annual seasonal influenza vaccinations unless otherwise exempted for medical or religious reasons.
This position may require immunization screening. Incumbent may be required to show proof of other immunizations depending on the type of position.

Basic Requirement for Behavioral Health Care Manager:

Degree: Bachelor's or graduate (or higher level) degree with major study in an academic field related to the medical field, health sciences or allied sciences appropriate to the work of the position. This degree must be from an educational program accredited by an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

Specialized Experience: One year of specialized experience which includes conducting behavioral health assessments/interviews; coordinating behavioral health care plans with a multidisciplinary team; and evaluating appropriateness, quality, or cost effectiveness of care. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-11).

Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone--no substitution of education for experience is permitted.

Education
