The Pharmacoeconomic pharmacist is a licensed pharmacist who plans, develops, coordinates, and manages all pharmacy and VA Northern California Health Care System (VANCHCS) activities, both clinical and administrative, directed at the containment and reduction of drug therapy costs, with particular emphasis on providing the most appropriate and cost-effective drug therapy to our veteran patients.

Duties include but are not limited to:

A key component of this position is evaluation of utilization trends and analysis of all cost factors associated with drug therapy to help the Pharmacoeconomic pharmacist identify opportunities for improving care and containing costs associated with the use of medications. Another key component of this position is the incumbent's ability to analyze drug therapy outcomes, including clinical, humanistic, and economic outcomes, from the implementation of specific programs aimed at improving prescribing.
The Pharmacoeconomic pharmacist will work in a multidisciplinary approach to set up and coordinate targeted utilization- and guideline-based educational programs. The Pharmacoeconomic pharmacist will help with the implementation of disease management systems for VANCHCS. The incumbent reports to the Associate Chief of Outpatient Operations.
a. Analysis of Prescribing Practices:
i. Prescription utilization data will be gathered from electronic databases within VISTA. Utilization data will be used to analyze patterns of drug utilization in order to identify potential drug use problem areas.
ii. Traditionally, the following steps are involved in determining prescribing practices:
1. Detection of a problem in prescribing through anecdotal reports, budget reports, adverse drug reaction trends, or detection of unusually high drug detailing.
2. Creation of institutional criteria to judge the correctness of prescribing medications.
3. Collection of data focused to evaluation of the criteria.
4. Analysis and interpretation of the data.
iii. Selection of drug or drug classes for study may also involve the classic drug use evaluation selection guidelines: high volume, high risks, and/or problem-prone drugs.
iv. Additionally, the analysis of prescribing practices may be linked to the functions of the Medication Use Evaluation (MUE) within the Pharmacy and Therapeutics (P&T) Committee. The pharmacoeconomic pharmacist will be an active member of the P&T Committee and may use guidance from the P&T Committee in the design of specific pharmacoeconomic programs.

b. Administrative:
i. Direct oversight of the VANCHCS Pharmacoeconomic clinical pharmacy program as well as the pharmacy Informatics and Procurement team. Direct oversight of all clinical pharmacy specialists and clinical pharmacy technicians providing care.
ii. Supervises subordinates, and ensures services provided meet or exceed the professional and legal standards. Oversees operation of the Pharmacoeconomic Clinical Pharmacy program, Informatics, and Procurement work assignments (individuals or groups), establishes priorities, and reviews work of subordinates.
iii. Communicates effectively, both orally and in writing, and performs required documentation in a timely manner.
iv. Maintains and submits statistics and other required reports in a timely manner.
v. Understands and adheres to policies and procedures of Pharmacy Service and VANCHCS.vi. Participate in all aspects of training such as conducting on-the-job training, conducting in-services, monitoring performance, and evaluating performance.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Education. (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy). [TRANSCRIPTS MUST BE SUBMITTED WITH APPLICATION](2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). [YOU MUST SUBMIT A COPY OF FPGEE AND TOEFL WITH YOUR APPLICATION]NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grandfathering Provision. All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
(1) Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions. (2) Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation. (3) If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.

Grade Determinations: GS-13

(a) Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
(b) Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.

Pharmacy Supervisor. Responsible for the professional and administrative management of an assigned area in pharmacy service, to include budgetary execution. Such individuals have responsibilities for supervising multiple pharmacists and technicians. Manages people, maintains effective interdepartmental relations, and cooperates with other services to accomplish the medical facility's mission and goals. Develops performance standards, assures that the program area is compliant with all regulatory and accrediting body requirements, designs and implements orientation and training programs for staff, and develops and maintains systems to monitor the performance of staff activities. Develops policies and procedures relative to their assigned area. Pharmacists assigned to this position must demonstrate the following KSAs:
a. Ability to effectively supervise subordinate staff.
b. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
c. Expert understanding of regulatory and quality standards pertaining to pharmacy.
d. Skill in managing people or programs.

The full performance level of this vacancy is GS-12. The actual grade at which an applicant may be selected for this vacancy is GS-13

Physical Requirements: This position requires visual acuity (depth perception and ability to distinguish basic colors and shades of colors), near vision correctable at 13 to 16, and far vision correctable in one eye to 20/20 and 20/40 in the other and keen hearing. This position requires potentially long periods standing (6 hours). The incumbent may be required to do light lifting and carrying of up to 15 pounds; reaching above shoulders. Both hands and legs are required along with the use of all fingers; hearing (aid permitted); repetitive computer use.

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
