Summary

The Home Based Primary Care (HBPC) Program Director is a Registered Dietitian; he/she is accountable for the management, supervision and evaluation of care for patients enrolled in the HBPC and other VA staffed in-home programs that are organizationally aligned under HBPC, which includes all facility and CBOC locations.

Help

Specific duties include, but or not limited to:

  • Visioning, planning, staffing, directing, budgeting, evaluating and tracking the programs, and developing VA and community relationships.
  • Accountable for developing, organizing, guiding, controlling, and evaluating programs and outcomes; daily attention to complex personnel management problems; participation in executive level decision making, and providing guidance and direction for the development and implementation of established standards of interdisciplinary practice.
  • Assuring appropriate resources and designation for the support of the HBPC Program and safety of the staff.
  • Direct supervision (or indirect oversight) and development of HBPC Supervisors.
  • Development and continued effective functioning of the interdisciplinary health care team; recognizing and effectively utilizing the skills, knowledge and contributions of each team member.
  • Collaborating with other HBPC Program Directors, VISN leadership, and VHA Central Office staff on issues of program development and operation.
  • Ensuring positive outcomes of existing programs and new projects within his/her service.
  • Providing leadership in managing programs to improve delivery of care, applies system redesign principles to improve clinical processes and enhance outcomes.
  • Assuring fiscal responsibility for staffing, productivity, workload capture, and material resources.
  • Participating in research activities and translates evidence into practice.
  • Ensuring compliance with the Joint Commission, OSHA and other regulatory agencies standards.
Work Schedule: Full-time, Monday - Friday, 8:00 am - 4:30 pm

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Registered Dietitian Nutritionist (RDN) or Registered Dietitian (RD) - Must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA).
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations:
Dietitian (Facility Program Coordinator), GS-13.
(1) Experience. One year of experience equivalent to the next lower grade level.
(2) Knowledge, Skills and Abilities. In addition to the experience above, the candidate must demonstrate the following KSAs:

(a) Knowledge of program administration to establish and modify program goals and objectives, coordinate activities, evaluate accomplishments and solve problems.
(b) Skill in developing quality improvement or research activities.
(c) Skill in developing training and reference materials.
(d) Ability to supervise subordinate staff (i.e., plan, organize, delegate, direct, control and review activities and performance).
(e) Ability to develop policies and procedures.
(f) Ability to provide program-specific coordination to include leadership, technical advice and staff development.

The full performance level of this vacancy is GS-13.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. May be required to complete annual Employee Health requirements, such as annual TB screening or testing, as employment conditions.
