Summary

Registered Respiratory Therapists at the GS-11 (full-performance) grade level work independently providing respiratory care in both inpatient and/or outpatient settings. They provide education to patients and train physicians, nurses and other clinical staff on the use of respiratory therapy equipment. They are knowledgeable of the more complex procedures related to the care of patients with respiratory issues.

Help

Duties at the GS-11 grade level, include but are not limited to:

  • Candidates must demonstrate the ability to collaboratively consult with physicians and other healthcare professionals for complex respiratory care to develop care plans to provide favorable patient outcomes.
  • Skills in evaluating the efficacies of therapeutic modalities.
  • Skills in performing advanced airway and ventilation management.
  • Performs the following procedures with proficiency and confidence under minimal supervision: Oxygen, aerosol, humidity, and positive pressure breathing therapy; Implement adult mechanical ventilation. Maintain, adjust, and monitor patients on a ventilator; Wean long-term ventilator patients; Performs weaning parameters on patients being weaned off a ventilator; Implement Acute and Chronic NIPPV set-up and use; Home NIPPV devices CPAP, BIPAP; Implement HFNC (High Flow Nasal Cannula) therapy.
  • Reviews the patient's chart and evaluates physician's orders for respiratory therapy.
  • Positions patient and performs chest physiotherapy and postural drainage therapy/positioning, noting effectiveness of therapy or performs Flutter Value or vest therapy.
  • Administers aerosolized medications as prescribed in the physician's order while following hospital policy regarding the administration of medication(s).
  • Must have basic understanding of blood gas and acid base interpretation
  • Consults with superiors and/or physicians on blood gas results and make recommendations for best course of action for the patient.
  • Responds to emergencies, make assessments, and implement proper actions and judgments.
  • Performs bedside pulmonary function testing and assist with bronchs as required.
  • May be called upon to aid in the evaluation of new equipment and supplies.
  • Performs all Respiratory Therapy tasks using aseptic techniques/universal precautions to provide best care available to the patient.
  • Follows department policies and procedures that have been approved and signed by the medical director of respiratory care services.
  • Assembles, cleans, and sterilizes equipment, as required.
  • Follows established Ventilator Associated Complications (VAC) and Acute Respiratory Distress Syndrome (ARDS) protocols and documents in CPRS/ICIP.
  • Actively participates in daily medicine/surgery rounds and provides recommendations on course of treatment as requested by the doctor.
  • Transports patients as needed to special procedures including but not limited to MRI or CT. Communicate with Nurse or Provider when problem arises and tries to work out an amicable solution.

Work Schedule: Tour consists of six 12-hour shifts and one 8-hour shift; rotating days including every other weekend.

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements.

  • United States Citizenship. Citizen of the United States (U.S.). Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, of this part. See 38 U.S.C. ยง 7407(a) for more information.
  • Education. Individuals must have successfully completed a respiratory care program accredited by the CoARC or its successor.
  • Licensure. Persons appointed or reassigned to RRT positions in the GS-0601 series must possess and maintain for the duration of employment a full, current and unrestricted license from a state to practice as an RRT.
  • Credential. Persons appointed or reassigned to RRT positions in the GS-0601 series must possess and maintain for the duration of employment a valid, current, unrestricted credential of RRT.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determination.

Registered Respiratory Therapist, GS-11
Experience, Licensure and Credential.
Candidates must have:
i. One year of creditable experience equivalent to the GS-09 grade level demonstrating the clinical competencies described at that level.
ii. Hold an active license and RRT credential.
Demonstrated Knowledge, Skills and Abilities. In addition to the requirements above, candidates must demonstrate all the following KSAs:
i. Ability to collaboratively consult with physicians and other healthcare professionals for complex respiratory care to develop care plans to provide favorable patient outcomes.
ii. Skill in evaluating the efficacies of therapeutic modalities.
iii. Skill in performing advanced airway and ventilation management.

Preferred Experience: Minimum of 3 years ICU experience in a high acuity facility (1A or comparable). Prefer experience with ALS/neuromuscular disease patient population.

The full performance level of this vacancy is GS-11.

Physical Requirements: Moderate lifting and carrying (15-44 lbs.), reaching above shoulder, use of fingers, both hands required, walking, standing, both legs required, ability for rapid mental and muscular coordination simultaneously, both eyes required, depth perception, and hearing (aid may be permitted). Environmental factors include radiant energy, working closely with others and working alone.
