Summary

The Phoenix VA HCS is seeking an Advanced Level Practice CDCES Dietitian to play a crucial role in the facility Diabetes Self-Management Education program (DSME). The individual will be responsible for working hand-in-hand with other DSME practitioners in providing comprehensive nutrition and diabetes education to individuals with diabetes and will be co- responsible for hosting the facility DSME classes and provide advanced level of care individuals with diabetes.

Help

Advanced Level Practice Dietitians are the subject matter experts in advanced clinical nutrition and uses knowledge, expertise, principles, and/or current research to lead dietitians and nutrition specialty care teams to plan, implement, and evaluate organizational initiatives. Typical duties include but are not limited to:

  • Conduct comprehensive nutrition and diabetes assessments.
  • Develop individualized care plans based on clinical guidelines, patient preferences, and lifestyle.
  • Provide DSME to educate patients on managing their diabetes effectively.
  • Teach patients about carbohydrate counting, insulin usage, blood glucose monitoring, diabetes technology, and the impact of nutrition on blood sugar levels.
  • Work in collaboration with healthcare teams, including endocrinologists, nurses, and other specialists to provide comprehensive care.
  • Ensure that all provided education and care meet the standards set by the American Diabetes Association (ADA) or the Association of Diabetes Care & Education Specialists (ADCES).
  • Staying current with emerging research and best practices in diabetes care and nutrition.
  • Provides consultative services regarding advanced therapies, policies, procedures, and research in the provision of quality nutritional care.
  • Develops, monitors, and directs practice-specific benchmarks based on standards of care or national initiatives.
  • Utilizes knowledge, expertise, principles and/or current research to plan, implement and evaluate organizational initiatives.

Work Schedule: Monday through Friday, 07:30am to 4:00pm but work schedule may be adjusted to meet the needs of the service.

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Registered Dietitian Nutritionist (RDN) or Registered Dietitian (RD) - Must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA).
Grade Determinations: Dietitian (Advanced Level Practice), GS-12.
  • Experience. One year of experience equivalent to the next lower grade level (GS-11).
  • Certification. Must hold an advanced practice certification from a nationally recognized certifying body such as the Commission on Dietetic Registration, the Association of Diabetes Care and Education Specialists or the National Board of Nutrition Certification. The certification must be directly related to the assignment being filled. For example, where the assignment requires diabetes management, a Board Certified-Advanced Diabetes Management or Certified Diabetes Care and Education Specialists certification is qualifying. Commonly held certifications include Board Certified Specialist in Renal Nutrition; Board Certified Specialist in Gerontological Nutrition; Registered Dietitian Nutritionist-Advanced Practitioner; Certified Nutrition Support Clinician.
  • Knowledge, Skills and Abilities. In addition to the experience and certification above, the candidate must demonstrate the following KSAs:
    1. Skill in leading a team in the implementation of nutrition-related activities.
    2. Skill in utilizing evidence- based practice guidelines to establish or modify goals and improve patient outcomes.
    3. Ability to communicate in writing to develop policies and standard operating procedures.
    4. Ability to serve as a nutrition expert, providing technical guidance, direction and staff development.
    5. Ability to lead research or performance improvement studies.
    6. Ability to present complex nutrition information to a large group of health care providers to persuade and influence provision of care.
Preferred Experience:
  • 3+ years providing diabetes care
  • Familiarity with Diabetes Self-Management Education program (DSME) protocols.
