Help

The incumbent of this position is responsible for promoting compliance with national, VISN, and facility formulary guidelines, resulting in appropriate and timely access to medications. Programs and program activities shall focus on pharmaceutical formulary selection, formulary implementation, medication conversions, high-risk medication management, medication use reviews, implementation/creation of ordering pathways, and staff education.

Duties of the position include but are not limited to the following:

Formulary Management Program Responsibilities:
The incumbent shall be responsible for the development and maintenance of formulary programs to include:

  1. Facilitate timely review of prescriptions from non-VA providers to include review for eligibility for non-VA care, and coordination with the non-VA provider to ensure compliance with formulary guidelines.
  2. Coordinate the timely evaluation of restricted and non-formulary medication requests, with consistent application of national, VISN, and facility guidelines
  3. Develop, implement, and manage programs to optimize drug therapy costs.
  4. Interface with internal and external customers to identify, develop, and implement appropriate formulary programs, outcomes assessment, and cost-saving initiatives.
  5. Monitors and trends pharmaceutical costs to identify best practices and opportunities for improvement.
  6. Plan and implement medication conversions to reduce drug therapy costs, while ensuring the availability of appropriate, effective therapy.
  7. Works with the Pharmacy Clinical Program Manager and Informatics Pharmacist to conduct prospective/ retrospective medication use reviews of high-use, high-cost, and/or high-risk drugs, including opioids. Works in conjunction with the Informatics Pharmacist to prepare and report analysis of data, make recommendations for corrective action, and develop education programs/interventions that promote rational, cost-effective prescribing.
  8. Coordinates monitoring and dispensing of high-cost and/or high-risk medications (to include oral chemotherapy, special order medications, and opioids) to ensure rational, appropriate use while preventing unnecessary waste.
Medication Therapy Management Activities:
The incumbent shall:
  1. Participate in the development /presentation of medication monographs and treatment protocols for the Pharmacy & Therapeutics Committee.
  2. Coordinate facility adverse drug event reporting, with appropriate submission to VA ADERS and FDA MedWatch.
  3. Documents all interventions following standard policies/ procedures for purposes of outcome, and workload assessment.
  4. Performs dispensing activities as needed. Screens medication orders for appropriateness regarding drug selection, dosing, allergies, drug-drug interactions, and laboratory information. Enters and /or verifies orders in VISTA and dispenses medications according to established policies/ procedures.
  5. Actively participates in the facility's Joint Commission continuous readiness program.
Education /Training:
  1. Works with the Pharmacy Clinical Program Manager to identify and target drug therapy interventions. Develops and performs in-services promoting cost-effective medication management to all health care providers and pharmacy staff within the Medical Center.
  2. Serves as a resource and provides information and education to pharmacy staff, providers, and patients. Education is provided through in-services, newsletters, and face-to-face encounters.
  3. Participates in the precepting of pharmacy students and pharmacy residents to increase the awareness and understanding of pharmacoeconomic and formulary management issues.
  4. Complies with all departmental and hospital professional/non-professional educational attendance and documentation requirements.

Work Schedule: Monday - Friday, 7:30am - 4:00pm
Duty Location: If selected, you will temporarily work at Jack C. Montgomery VA Medical Center at 1011 Honor Heights Drive, Muskogee, OK 74401 until the completion of the new hospital in Tulsa, at which point your duty station will change to the new Tulsa location, James Mountain Inhofe VA Medical Center, 440 South Houston Avenue, Tulsa, OK 74127.

Qualifications

Basic Requirements:

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

Education.
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet- Based Test (TOEFL iBT).

Licensure.
Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.

GS-13
Experience.
In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level which includes:
1. Knowledge of professional pharmacy practice.
2. Ability to communicate orally and in writing to both patients and health care staff.
3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Clinical Pharmacy Specialist.
The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.

Pharmacists assigned to this position must demonstrate the following KSAs:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Preferred Experience:

  • Preferably 1+ years of pharmacy experience at EOVAHCS. Strong knowledge of pharmacy operations, clinical pharmacy practices, and medication management.
  • Preferred experience with electronic health record (EHR) systems and pharmacy management software.
  • Knowledge of contemporary hospital/clinical practice and service. Significant knowledge and expertise in advanced pharmacy practice, specialty medication therapy management, and specialty evidence-based medicine.

The full performance level of this vacancy is GS-13. The actual grade at which an applicant may be selected for this vacancy is GS-13.

Physical Requirements: Moderate or extended periods of sitting, standing, walking, bending, stooping, stretching, reaching, lifting, pushing, and pulling. This occasionally involves lifting, moderately heavy items (30 pounds). The employee must possess average agility and dexterity.
