CLINICAL:

  • Provide timely screening, psychosocial assessment, and diagnoses of mental disorders.
  • Develop, implement, and document the psychosocial assessment, treatment plan, progress notes and updates, and discharge planning for provision of clinical services to Veterans, including prompt completion of clinical reminders.
  • Provide psychoeducation to Veterans and family members about mental health conditions and treatment methods.
  • Provide consultation to health care providers about assessment findings and treatment plans for Veterans.
  • Conduct: routine suicide risk screens; comprehensive suicide risk evaluations, and collaborative safety plans when clinically indicated; follows-up with Veterans who fail to attend appointments per local procedures; seek needed consultation with the Suicide Prevention Coordinator, Disruptive Behavior Committee, and/or CHOS Program Manager, as needed, to ensure the safety and welfare of Veterans; remain up to date on suicide prevention training materials and clinical tools and utilize them as appropriate in clinical practice.
  • Available to aid and problem-solving to provide excellent customer service through additional support, providing further clarification on diagnosis, eligibility criteria, program rules and/or regulations, access to care/resources, etc.
  • Provide consultation and supervision to social work trainees and unlicensed staff as indicated.
  • Provide leadership in identifying and addressing ethical issues that impact patients and staff, including initiating and participating in ethical consultations. Maintain complete patient confidentiality. Analyze ethical/care issues, applying appropriate theoretical principles and ensures application of institutional policy. Serve as a resource and advocate for the protection of patient/family rights. Provide considerate care, respecting patient/family and staff personal values and belief systems.
  • Participate in regularly scheduled clinical consultations with staff/treatment teams to ensure for quality and continuity of care, as well as individual Veteran attainment of program and treatment goals.
  • Function as a liaison for coordinating outreach efforts with local community partners; including ensuring linkages to VA services and the local SPC for those Veterans who are experiencing literal street homelessness and are at high risk for suicide and participating in case conferencing and other pertinent community meetings as needed.
  • Serve as a direct link between community services and CHOS homeless services through regular and consistent outreach and communication with community partners.
  • Performs other related duties as assigned.
ADMINISTRATIVE:
  • Provide training and supervision of a multidisciplinary team.
  • Monitor compliance of staff members with VA performance measures and effectively shapes provider practice patterns to improve standing on performance measures.
  • Lead and facilitate quality improvement projects.
  • Develop partnerships with community agencies and maintain regular contact and communication.
  • Ensure accuracy of staff's data collection in VA Homeless Operations Management and Evaluation System (HOMES) or other database systems as directed by the VA Central office and electronic medical charting documentation. Perform a regular review of caseloads to ensure appropriateness of acuity assignment of each Veteran in HOMES and well as ensuring for as balanced a caseload (acuity and numbers) as possible to each staff member supervised.
  • Monitor and manage electronic medical homeless consults for geographical area.

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Qualifications

BASIC REQUIREMENTS
1. United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

2. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited.

A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.

3. Licensure: Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.

GRADE DETERMINATIONS
Supervisory Social Worker, GS-12

1. Education and Experience. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills in a specialty area or in administration demonstrating progressively more professional competency and judgment. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

2. Licensure/Certification. Individuals assigned as Supervisory Social Workers must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

3. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Ability to independently organize work, set priorities, and meet multiple deadlines.
(b) Knowledge of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.
(c) Ability to ensure provision of clinical social work services.
(d) Ability to provide staff training and development.
(e) Skill in interpersonal relationships in dealing with employees, team leaders, and managers.

PREFERRED EXPERIENCE: Experience and knowledge of working with people who have experienced homelessness. Leadership experience (leading projects or supervision experience).

The full performance level of this vacancy is GS-12.

PHYSICAL REQUIREMENTS: Moderate lifting & carrying (15-44 pounds), pushing (6 hours), operation of a motor vehicle, ability to distinguish basic colors, and hearing (aid may be permitted). Environmental factors include outside and inside, working closely with others, and working alone.
