Duties

Help
  • Provides proactive intervention in discharge planning activities for inpatients.
  • Monitors patient compliance with treatment plan and identifies obstacles to positive outcomes.
  • Provides referrals and counseling to patients regarding access to care, options for resolution of complex psychosocial issues.
  • Serves as consultant to staff and providers on psychosocial issues, family dynamics and adjustment to illness.
  • Provides one-on-one assessment, intervention and appropriate referrals.
  • Coordinates outpatient case management services for designated patient population.

  • Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.

  • Must be able to obtain and maintain current Basic Life Support (BLS) certification through the American Red Cross. Advanced certification (e.g., ALS) does not supersede BLS.
  • Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.

  • This is a drug testing designated position. Incumbent agrees to submit to a mandatory command directed urinalysis on a recurring basis IAW AR 600-85.

In order to qualify, you must meet the education and experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience. Basic Requirement for Social Worker:

Degree: Master's degree (or higher degree) in social work from a school accredited by the Council on Social Work Education.

In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

Experience required: To qualify based on your experience, your resume must describe at least one year of experience which prepared you to do the work in this job.

Specialized experience for the GS-11 is defined as: Performing psychosocial interventions, consulting with other resources to coordinate a plan of care, and developing treatment plans. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-09).

Some federal jobs allow you to substitute your education for the required experience in order to qualify. For the GS-11, you may qualify if your education meets the definitions below:

Education Substitution for Specialized Experience at GS-11: Successful completion of a Ph.D. or equivalent doctoral degree, or three full years of progressively higher level graduate education leading to such a degree, that is directly related to the work of the position.

Combination of Experience and Education Substitution for Specialized Experience for GS-11: If you have some, but not all, of the experience or education described above, you may still qualify by combining the amount of creditable experience and education that you possess. To compute the percentage, divide your total months and education that you possess. To compute the percentage, divide your total months of qualifying experience by 12. Then divide your semester hours of graduate education in excess of 36 hours (total semester hours minus 36) by 18. Add the two percentages. The total percentage must equal at least 100% to qualify.

Licensure: A current, active, valid, unrestricted clinical license to practice Social Work independently, offered by a U.S. State, District of Columbia, Commonwealth, territory, or jurisdiction is required.

Education

FOREIGN EDUCATION: If you are using education completed in foreign colleges or universities to meet the qualification requirements, you must show the education credentials have been evaluated by a private organization that specializes in interpretation of foreign education programs and such education has been deemed equivalent to that gained in an accredited U.S. education program; or full credit has been given for the courses at a U.S. accredited college or university.
