Summary

This position is located within the Department of Homeland Security (DHS), Immigration and Customs Enforcement, Enforcement and Removal Operations, ICE Health Service Corps, Office of Deputy Assistant Director (DAD) for Clinical Services Division, IHSC Clinical Services Support Unit (CSSU), Infectious Disease Program. The incumbent will report to the Chief of the CSSU. This is a supervisory position.

This position is only open to USPHS officers and Call to Active Duty (CAD) candidates.

DUTIES AND RESPONSIBILITIES:

  1. Recognized subject matter expert in infectious disease and transgender care, acting as liaison with partner agencies and stakeholders (e.g. Customs and Border Patrol, Bureau of Prisons, U.S. Marshals Service, Centers for Disease Control and Prevention, local health departments, international partners) as well as DHS headquarters to optimize surveillance, continuity of care, guideline development and optimal disease management.
  2. Provides clinical oversight through direct patient care and clinical consultation for infectious diseases and transgender care in IHSC facilities.
  3. Provides clinical expertise to the IHSC Public Health Safety Preparedness Unit including: immunizations, antibiotic stewardship and minimizing development of drug resistance.
  4. Leads outbreak investigations and containment.
  5. Directs pandemic planning and response.
  6. Coordinates infection surveillance efforts.
  7. Provides diagnostic and clinical management guidance to IHSC clinicians directly or via direct supervision of the ID Advanced Practice Provider, ID Clinical Pharmacist and Public Health Analyst for noncitizen patients with various infectious diseases including but not limited to HIV, tuberculosis, viral hepatitis, syphilis and other sexually transmitted infections, skin and soft tissue infections, respiratory infections, enteric infections, ectoparasite infestations, and tropical infections (e.g. helminths, malaria, leishmaniasis, etc.) as well as transgender care.
  8. Provides clinical expertise in development of the IHSC formulary with respect to antimicrobial agents.
  9. Conducts clinically and policy-relevant surveillance and research in support of IHSC mission.
  10. Advises on facility planning and environmental design review.
  11. Advises on quality assurance programs for deployment at field level.
  12. Develops and revises clinical practice guidelines and electronic health record tools involving infectious disease and transgender care for IHSC staff and patients.
  13. Reports and presents relevant surveillance and programmatic findings at agency approved venues.
  14. Provides remote and/or on site continuing medical education on relevant infectious disease and transgender issues to IHSC as well as community partners.
  15. Will serve as a subject matter expert on agency committees including but not limited to the Pharmacy and Therapeutics Committee, Antimicrobial Stewardship Committee, Ethics Committee, Tuberculosis Coordination and Care Team and the HIV Coordination and Care Team.
  16. Additional duties as assigned by the Chief of the Clinical Services Support Unit or designee.
  17. Perform other duties and responsibilities as assigned.
  18. Occasional travel required to support IHSC facilities.

Qualifications

PHYSICAL DEMANDS:

  1. Must be able to perform duties in a fast-paced environment without physical limitations.
  2. Sitting and/or standing for extended periods of time [6-8 Hours].
  3. Performing repeated bending motion.
  4. Average manual dexterity for computer operation.
  5. Phone/laptop use for telephonic/virtual meetings for extended periods of time.

Education

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

  1. Doctor of Medicine or Doctor of Osteopathy from a school in the United States or Canada approved by a recognized accrediting body in the year of the applicant's graduation.
  2. Doctor of Medicine or equivalent degree from a foreign medical school that is certified by the Educational Commission for Foreign Medical Graduates (ECFMG).
  3. Evidence of board eligibility/certification is required in an appropriate primary care specialty (Internal medicine) by an American Board of Medical Specialties (ABMS) or American Osteopathic Association (AOA) approved board.
  4. Formal fellowship training in infectious disease with or without board certification is preferred.
  5. Current, unrestricted, and valid medical license from any U.S. state, Washington D.C., Puerto Rico, U.S. Virgin Islands, or Guam.
  6. Minimum of 3 years practicing in clinical infectious disease beyond fellowship training or a minimum of 5 years beyond internal medicine residency (if no formal infectious disease training).
  7. Knowledge of the principles of public health and epidemiology.
  8. U.S. Public Health Service Officers must comply with and obey all federal rules, regulations, policies, guidance and established procedures that includes professional competence, readiness and, suitability.
  9. As applicable, incumbents must satisfy continuing education requirements must be met as required for licensing and board certification maintenance.
  10. Maintains current CPR/Basic Life Support Certification.
  11. Interact effectively and collaboratively with interdisciplinary teams to ensure the provision of medical care of individuals in ICE custody and development of program initiatives.
  12. Strong communication skills with demonstrated ability to clearly and effectively communicate both orally and in writing.
  13. Demonstrated ability to maintain confidentiality of information; exercise judgment and discretion in handling and disseminating information.
  14. Ability to grasp conceptual ideas and use critical thinking skills in situations where consultation and evaluation are needed to offer sound suggestions/guidance; provide guidance/suggestions potentially with short notice.
  15. Demonstrated initiative, ability to function with high degree of independence, and follow-through on taskings, projects and other work commitments.
  16. Demonstrated evidence of leadership skills.
  17. Demonstrated ability to promote, build and maintain a collaborative team environment.
  18. Professional background in health care with direct patient care experience, preferably within a correctional/detention setting.

SUPERVISORY CONTROLS:
The incumbent reports to and works under the supervision of the Chief CSSU.
The employee receives general direction from the Chief CSSU, or from the DAD CSD, who outlines assignments and projects, establishes timeframes, and provides guidelines for scope of the assignment and possible approaches for completion of work. The employee independently develops work plans for the accomplishment of assigned objectives, carries out, assignments and advises the supervisor of changes, precedent setting, or controversial issue.

The incumbent establishes clear goals and objectives, implements, and maintains the agency's operations to promote integrity of the IDP. Serves as a first line supervisor for IDP clinical staff and public health analyst and has responsibility for distribution of workload, and usage of program resources. Involved in all aspects of the administration of the IDP and works independently; critically involved in the resolution of issues within the IDP problems.
The employee plans and organizes his/her own work, determines the sequence of assignments, selects, and develops methods, and seeks assistance from experts only rarely; works independently to resolve complex and unusual issues, through research, analysis and action; assists with development of conclusions and recommendations necessary to implement the objectives and policies of the DHS Office of Health Affairs and the IHSC DAD CSD. The incumbent uses initiative in carrying out assignments and independently develops solutions to deviations, problems, or unusual situations. The incumbent assignments are usually long-term, recurring, or broadly defined. The supervisor reviews completed work for soundness of overall approach, effectiveness in meeting requirements or producing expected results, the feasibility of recommendations, and adherence to requirements, and compliance with policies pertinent regulations, civilian accreditation agency requirements and standing operating procedures.

The incumbent directs a program of agency level and of nationwide scope and importance; programs directed are essential to the integrity of clinical care provided by the agency at ICE detention facilities across the nation. The IDP Lead may serve in the absence of the Chief CSSU, with information related to provision of high quality clinical care across the nation, and to ensure quality measures are in place for the IDP. The officer analyzes results and recommends changes in guidelines and program policies. Innovations and modifications to accepted procedures must be approved and clearly documented. The officer keeps the supervisor informed of the status on projects and consults with him/her on any significant problems encountered.

A moderate number of guidelines may be applicable to some parts but not all parts of assignments and may contain some inconsistencies and be partially unconfirmed. The incumbent uses judgment in interpreting and adapting guidelines such as agency, national, PHS policies, regulations, precedents, and work direction for application to specific cases.
