Summary

This Social Worker position is for the Contract Nursing Home (CNH) and Contract Adult Day Health Care (CADHC) Programs and monitors clinical aspects of the programs across the Milwaukee VAMC and related Community Based Outpatient Clinics (CBOC's). This position is located within Community Based Services (CBS), in the Rehabilitation Extended and Community Care Division. The Duty location of this position is located at the Green Bay CBOC.

Duties

Help

  • Responsible for completing monthly monitoring of CNH and quarterly monitoring of CADHC facilities and follow-up to Veterans in the CNH and CADHC Programs.
  • Assists with the completion of annual VA CNH program reviews and should have a working knowledge of Centers for Medicare/Medicaid standards concerning nursing home programs and state and VA standards regarding adult day care programs.
  • Assists with ensuring that quality assurance related to these programs is within program parameters.
  • Assists with the maintenance of a current list of active participants in the programs to enable tracking of service activities, utilization of services, and other functions.
  • Participates as a member of the CNH and CADHC inspection teams, assisting with site visits for new contracts, or follow up to reports of care concerns.
  • Provides oversight of care to veterans in receipt of VA contract nursing home care and contract adult day health care.
  • Completes contract nursing home and day care chart reviews and conducts regularly scheduled onsite visits to veterans for assessment of overall quality of care.
  • Serves as a liaison between VA contract facilities and the VA Medical Center, and provides consultation to contract nursing home and day care staff as needed or requested to ensure appropriate medical services and quality of care.
  • Establishes and maintains effective therapeutic relationships with veterans, family members and other caregivers.
  • Independently works with patients and families who are experiencing a variety of psychiatric, medical and social problems.
  • Provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to treatment plan.
  • Evaluates incumbents own practice through professional peer review, case conferences and other organizational means.
  • Knowledge of veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension. Facilitates referrals based upon veterans needs and eligibility.
  • A substantial portion of the workload directed by this position is carried out at one or more locations that are physically removed from the main facility, which may make day-to-day supervision difficult to administer, requiring a high degree of autonomy.
Preferred Experience
  • Two years experience working in long term care or geriatrics.
Driving will be Required in this position and a valid Drivers License is required for this position.

Pay: Competitive salary and regular salary increases . When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: Paid annual leave based on hours worked - accrual starting on your first day. Paid annual sick leave based on hours worked - accrual starting on your first day (no limit on accrual)
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Retirement: Traditional federal pension (5 years vesting) and federal 401K (3 years vesting) with up to 5% in contributions by VA
Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Rates will be prorated or based on hours worked.

Qualifications

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j.
  • Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED)
  • Licensure: Must be licensed or certified by a state to independently practice social work at the master's degree level. (COPY OF LICENSE REQUIRED)

In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates.

Grade Determinations:

GS-11 Level: In addition to the basic requirements, you must have a minimum of one year of post-MSW degree experience in the field of health care social work or other social work-related settings (VA or non-VA experience). Examples of specialized experience include but are not limited to:
  • Working under close supervision in program areas that do not require specialized knowledge or experience.
  • Identifying behaviors or symptoms of abuse, neglect or exploitation
  • Providing education on advance directives and advanced care planning
  • Providing social work case management
  • Acting as an advocate with community service providers/agencies when it serves the best interest of the patient and family members/caregiver;
  • Assessing the psychosocial functioning and needs of patients and their family members identifying strengths, weaknesses, coping skills and psychosocial acuity, in collaboration with the patient, family, and interdisciplinary treatment teams
  • Maintaining a current network of internal and external resources to educate the patient and/or family members/caregivers and assist with the appropriate referrals.


OR

Doctoral degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE) as this may be substituted for the required one year of professional social work experience in a clinical setting. (TRANSCRIPTS REQUIRED)

AND

Candidates must demonstrate all of the following Knowledge, Skills and Abilities:
  • Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
  • Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.
  • Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
  • Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.
  • Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment.

The full performance level of this vacancy is GS-11.
