Duties and responsibilities include, but are not limited to:

  • Participate on and guide an interdisciplinary team consisting of a Registered Nurse case manager, Occupational Therapist, Physical Therapist, Speech-Language Pathologist, Psychologist, Physiatrist, and Prosthetist/Orthotist, Recreational Therapist and Blind Rehabilitation Specialist.
  • Develop and implement realistic and achievable social work treatment plans, as part of the interdisciplinary treatment plans that reflect an ability to effectively utilize a broad range of treatment modalities.
  • Advocate on behalf of the Service Member/Veteran to ensure that services and benefits are obtained in a timely manner and in keeping with the VA's goal for excellence in customer service.
  • Coordinate outreach efforts to identify eligible Level II returning combat Service Members.
  • Collaborate with VISN POC, VHA Facility POCs, VA-DOD Social Work Liaisons, military treatment facilities and community agencies to disseminate information and develop resources accordingly.
  • Adhere to Service Line Agreements (SLA) and Standard Operating Procedures (SOP) for documentation. Adhere to approved templates and required standards for documentation as outlined by accrediting bodies such as Joint Commission, CARF, and/or revenue sources providing funding to VHA.
  • Provide documentation sufficient to meet the needs of various interdisciplinary teams engaged in the discharge planning process both within the facility and for potential placement and/or aftercare programs/facilities.
  • Comply with SLA, SOP, and directives for clinical contact with patients, families, and collateral providers over the course of treatment, which includes pre-admission, program participation, and post discharge follow up.
  • Provide individualized care coordination and discharge planning highlighting patients strengths, needs, abilities, and preferences within patient interaction, Interdisciplinary Team (IDT) meetings, along with family and collateral contacts.
  • Advise supervisor and IDT on identified treatment goals, priorities, and disposition options and progress.
  • Ensure patients have access to care via completion of required documents, 1010s, Advanced Directive, and other forms that assist with access to care within the VA or the community.
  • Participate in survey accreditation(s) ensuring standards are adhered to within their social work practice, unit/service and at the facility level as well (i.e. infection control and safety).
  • Ensure reports, PI projects, and other documentation required for accreditation are current and up to date.
  • Attend all assigned supervisory meetings to include individual, team huddles, and will participate in, both social work and matrix, staff meetings. The incumbent will participate on all assigned committees, work groups within or external to the facility.
  • Provide coverage for social work services on the assigned unit/service, and when needed for other social work teams inpatient and outpatient as needed.

Work Schedule: 7:30am - 4:00pm, Monday - Friday

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
  • Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.

GS-12 Senior Social Workercandidates must have the following:
  • Experience/Education: The candidate must have at least two (2) years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one (1) year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
  • Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure.

These are the KSA's for this subject position:
  1. Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
  2. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
  3. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
  4. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
  5. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.

Preferred Experience:
  • Case Management experience working with Veterans and Active Duty Service Members.
  • Medical and mental health clinical experience needed for completion of bio-psychosocial assessments, implementation of treatment interventions, and determination of appropriate level of care.
  • Knowledge of community resources is required for disposition and discharge planning purposes.
  • Crisis intervention skills and the ability to remain calm in high pressured situations is mandatory to effectively problem solve and implement effective clinical interventions.
Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive/operate government vehicle.

Receiving Service Credit or Earning Annual (Vacation) Leave: Federal Employees earn annual leave at a rate (4, 6 or 8 hours per pay period) which is based on the number of years they have served as a Federal employee.
