Duties include but not limited to:

Administrative:

  • Provides direct supervision to the pharmacists, technicians, students, and volunteers assigned to the outpatient pharmacy. The incumbent will approve or disapprove leave requests, write functional and position descriptions and performance standards, evaluate employees, formulate work schedules, recommend personnel actions.
  • Interviews employee candidates, recommends appointments, special advancements, within-grade increases, or disciplinary actions to the Professional Standards Board or the Chief, Pharmacy Service as appropriate.
  • Manages the daily operation of the outpatient pharmacy including the window and mail prescription dispensing, repackaging, and extemporaneous compounding.
  • Oversees day-to-day maintenance of operation of pharmacy equipment and automation. Utilizes available reporting functions to optimize the efficiency of this equipment.
Program Management:
  • Manages and ensures optimum utilization of all resources (personnel, fiscal, equipment, space, etc.) available to the outpatient pharmacy section.
  • Manages and ensures optimum efficiency and effectiveness of the numerous complex, and comprehensive programs administered by the outpatient pharmacy section.
  • Coordinates prescription filling for all fee basis providers in private sector healthcare systems and gives feedback to them when clinical or formulary conflicts arise.
  • Program manager for interactions between the outpatient pharmacy and the consolidated mail-out pharmacy (CMOP) including CMOP rejects and medication errors.
Performance Monitoring:
  • Responsible for initiating and maintaining performance improvement activities, in order that pharmacy processes may be continually evaluated for opportunities for improvement.
  • Chairs, prepares Agenda and minutes when assigned for the Pharmacist and Technician staff meetings.
  • Improve the quality of medical care for patients through promotion of better medication management. Promote active intervention in medical care where appropriate.
  • Actively participates or leads a process improvement team or a committee when assigned.
Pharmacy Practice:
  • Enters or completes provider orders in VISTA for medications in assigned area, carefully screening each order for therapeutic appropriateness and policy compliance.
  • Consults orally and in writing with other health-care providers regarding drug therapy selection and management. Resolves all medication related problems in a timely manner.
  • Participates as a member of the health care team in an interdisciplinary approach to provide quality patient care.
  • Acts as the pharmacy's patient representative to resolve patient concerns. Serves as a professional role model to subordinates and coworkers in all activities.
Quality/Safety:
  • Assures compliance with outpatient pharmacy standards articulated by the Department of Veterans Affairs as well as those identified by appropriate external review organizations, including the JCAHO and OIG.
  • Coordinates all medication safety initiatives pertaining to the outpatient Pharmacy including adverse drug event and medication error reporting. Identifies potential medication safety hazards, develops plans to improve medication safety, and implements changes to reduce medication errors.
  • Develops and maintains performance improvement and quality assurance monitors for pharmacy service provided for outpatients. Reports conclusions, actions, and impact to higher management.
Clinical:
  • Participates in pharmacy-based drug information services and provides drug information to health care providers.
  • Assures the accuracy and appropriateness of drug dispensing based on patient history and medication profile review. This includes screening prescriptions for drug-drug and drug food interactions, dosing errors, allergies and final verification of the order. Consults with other health care providers, as required. Enters prescriptions into the data-base and serves as final verifier of order entered.
  • Consults patients and other health care providers on the appropriate use of medication and provides appropriate written information, as required.
  • Assists in the development and maintenance of pertinent outpatient clinical pharmacy programs from cost and patient outcome perspective.
Education/Research:
  • Coordinates pharmacist and technician staff development.
  • Coordinates training of new employees, residents, and students in outpatient areas.
  • Coordinates training and work activities of volunteers.
  • Identifies continuing education needs based on staff performance and pharmacy goals.
  • Precepts pharmacy residents and students.

Qualifications

Basic Requirements:

  • Citizenship: Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g, this part.)
  • Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://vvww.acpe-accredit.orgt (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
  • English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
  • Grandfathering provision: May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations: In addition to the basic requirements for employment in paragraph 2, the following criteria must be met when determining the grade of candidates:

GS-12 Pharmacist (Full Performance Level)
  • Experience or Education: In addition to the basic requirements, candidates must meet one of the following:
    • 1 year of experience equivalent to the next lower grade level; or
    • Completion of an ACPE-accredited Pharm.D. Program.
Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
  1. Knowledge of professional pharmacy practice.
  2. Ability to communicate orally and in writing to both patients and health care staff.
  3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
  4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
GS-13 Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.

Pharmacists assigned to this position must demonstrate the following KSAs:
  1. Ability to effectively supervise subordinate staff.
  2. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
  3. Expert understanding of regulatory and quality standards pertaining to pharmacy.
  4. Skill in managing people or programs.
Preferred Experience: 2 years experience in a supervisory or leadership role. PGY 1 and/or Ambulatory care PGY 2 Pharmacy Residency completion or equivalent experience.

The full performance level of this vacancy is GS-13.

Physical Requirements: The work requires regular and recurring physical exertion. The physical demands of the work includes moderate lifting/carrying of pharmaceutical supplies weighing up to 44 lbs., pushing and pulling of large medication carts, light lifting of under 15 pounds, reaching above shoulder, use of fingers, good dexterity (both hands required), walking up to 4 hours, standing or sitting for prolonged periods of time, kneeling, repeated bending/stooping, climbing, use of legs and arms, operation of vehicle/motorized cart, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, far vision correctable in one eye to 20/20, specific visual requirement, ability to read fine print on labels, depth perceptions, ability to distinguish basic colors, ability to distinguish shades of colors, hearing (aid permitted), clear speech, and emotional stability.
