Summary

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Major duties include, but are not limited to, the following:

  • Assesses and re-assesses patients who are consulted to REC using appropriate tests/measures, evaluates findings, and develops a plan of care with functional and measurable goals within an appropriate timeframe that are based on assessment findings.
  • Selects, implements and interprets appropriate outcomes measures for each patient.
  • Utilizes current evidence based physical therapy practice to guide patient care.
  • Provides expertise in planning for delivery of physical therapy to patients. Acts as a resource person to the staff, students, and other health care professionals.
  • Exercises highly skilled autonomous professional judgment to establish appropriate discharge planning recommendations to ensure safe discharge.
  • Demonstrates the competency to treat and provide education to young adults across the lifespan to include geriatric patients and refer to other medical professionals as medically indicated.
  • Demonstrates the teaching skills required to provide education to patients, family members, caregivers, employees, students, and coworkers.
  • Functions autonomously in carrying out patient education, specialized treatments, and applying physical therapy procedures for which competency has been established and in accordance with laws or regulations and APTA documents.
  • Within scope of practice, implements and modifies treatment plan, therapeutic activities and interventions based on the changing needs, goals and performance of the patient as necessary to assure optimum patient outcomes.
  • Incumbent demonstrates ability to make autonomous clinical decisions in a Direct Access environment. This includes ability to independently evaluate and treat patients who seek physical therapy services.
  • Completes accurate and effective gatekeeping to determine eligibility for veterans referred for power mobility and custom manual wheelchairs.
Following current VA eligibility requirements, and requiring little to no supervision, performs specialized and detailed assessments and interventions to make recommendations for complex power mobility and special seating systems. The assessment and intervention may include, but is not limited to:
  • Determining the reason for referral
  • Clarifying the primary problems as the focus of the seating and mobility assessment
  • Differentiating among the veteran's needs, desires, concerns, abilities, function, potential, limitations, risks and precautions
  • Explaining to the veteran the components of the evaluation process (e.g., assessment of posture, skin, mobility, function as well as product trial, specifications, funding, fitting and training process)
  • Assimilating relevant information from the medical record, interviews with veteran/caregiver, and other team members to identify needs and goals (including contacting external professionals)
  • Reviewing current seating and mobility devices used, discussing the pros and cons of these devices
  • Analyzing veteran's current level of activities and participation and eligibility for device
  • Performing or assisting in a physical assessment (determining current seating posture, muscle tone and strength, joint/muscle flexibility, skin integrity, ability to weight shift, vision, cognition, dexterity, etc.) including assessing veteran's current functional status to determine eligibility for VA to provide device
  • Performing a functional seating assessment to determine optimal support needed to maximize function, comfort and posture
  • Performing an equipment assessment/review of potential power or manual mobility product options in varied environments
  • Utilizing appropriate seating and mobility related tests and measurement tools (e.g., pressure mapping, balance testing, fall-risk assessment)
  • Consulting and collaborating with appropriate professionals and resources as part of a complete seating and mobility assessment
  • Participating in the fitting and set-up of the prescribed devices (when available)
  • Assessing the veteran's ability to utilize the recommended devices in the intended environment
  • Recognizing consumer dissatisfaction or change of status, equipment failure, or other poor outcomes, and taking appropriate action to improve and optimize the intervention in conjunction with other members of the team.
This position requires the Program Coordinator to travel to offsite locations (institutions, including OSVH, CBOCs, etc.) to provide access to services for vulnerable veterans (when travel is too burdensome for the veteran).

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: Must be proficient in spoken and written English in accordance with VA Handbook 5005
Education and Experience: The individual must meet at least one of the requirements below:
  • Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. OR
  • Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. OR
  • Doctorate degree in physical therapy.
Licensure: Individuals must hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations: GS-13
Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level, the candidate must demonstrate the all following KSAs.

Demonstrated Knowledge, Skills, and Abilities:
  1. Ability to apply advanced knowledge of contemporary physical therapy within specialty area to provide clinical guidance, training or education for internal and external stakeholders.
  2. Ability to serve as a consultant within the assigned program area for intra-agency and interagency planning and to provide service coordination to optimize program continuity, efficiency and effectiveness.
  3. Knowledge of and skill in management, administration and/or education methodologies. This includes the ability to monitor and track data, utilize available resources effectively, evaluate program quality, and generate reports for local, and/or VISN and/or VACO leadership.
  4. Ability to develop and coordinate treatment and/or educational programs within a concentrated field or specialty area, such as, but not limited to, amputee, spinal cord injury, chronic pain, student clinical education/residency programming, polytrauma/traumatic brain injury.
  5. Ability to establish and maintain collaborative relationships within department and with other departments or affiliated programs.
  6. Ability to guide the work of a multi-disciplinary team.
