Summary

The incumbent is responsible for providing pharmaceutical care to assigned long term care areas and Home Base Primary Care (HBPC) as directed by the Clinical Pharmacy Supervisor or Associate Chief of Pharmacy for Clinical, The Pharmacist will work independently an in consultation with physicians and other health care providers to ensure appropriate medication therapy for individual patients. Clinical:

  • Provides a comprehensive drug therapy review every 30 days on all long-term care patients and every 90 days on all Home Based Primary Care (HBPC) patients to assess drug use patterns, compliance, therapeutic duplications, significant drug interactions, appropriateness of drug therapy and patient care outcomes.
  • Designs, implements, monitors, evaluates, and modifies patient pharmacotherapy to ensure effective, safe, and economical patient care. Completes consults as requested to assess pharmacotherapy.
  • Provides the full range of resources as a specialist in drug information covering a wide range of therapeutic classes especially in the area of his/her specialty training. Advises other members of the medical/surgical team as well as other health care providers on all matters relating to medication, i.e., drug administration, toxicities, incompatibilities, dosages, alternative medications, contraindications, drug interactions, formulary medications, etc.
  • Participates in taking medication histories. Screens these patients' medication profiles for adverse drug reactions and records in Computerized Patient Record System (CPRS) chart as well as VA Adverse Drug Event Reporting System (ADERS) , compliance, drug interactions, drug allergies and drug-food interactions. Investigates and reports any suspected adverse drug reactions (ADRS). Counsels' patients to improve patients' compliance with treatment plan.
  • Keeps accurate records of clinical interventions for later follow-up and quality assurance purposes.
Order Entry:
  • Develop and maintains a working knowledge of VISTA/CPRS software package and serves as a resource for others. Ensures all orders he/she enters are accurate and timely.
  • Documents clinical findings, treatment plans, etc. using the electronic medical record.
  • Records medication-related information and consultation in the medical record.
Education:
  • Prepares and presents educational lectures to other medical staff, staff pharmacists, pharmacy residents, and pharmacy students as needed. Teaches others of approved treatment guidelines and criteria to increase appropriate use of medications.
  • Serves as preceptor to clinical pharmacy students and residents in hospital pharmacy or clinical rotations, where applicable.
  • Evaluates such students and residents based on performance, presentations, and exams.
  • Participates in patient education programs and instructs both inpatients and outpatient in the proper use of their medications.
  • Maintains knowledge base of medications and disease states common to adult VA populations.
Research:
  • When assigned he/she designs and/or participates in research studies (including medication use evaluations) that involve patients assigned to his/her areas.
  • When assigned he/she maintains informational files on the chemistry, pharmacology, toxicology, biopharmaceutics, and pharmacokinetics of all investigational medications that are used by patients assigned to his/her area.
Program Management/Administrative Functions:
  • Assists in developing and updating various medication-related policies and operational procedures that affect his/her area of practice.
  • Maintains an organized, structured, long-term care/ HBPC clinical rotation for pharmacy students and residents.
  • Plays a major role in achieving good patient care outcomes at a reasonable cost.
  • Representation of Pharmacy Service on requested committees.
  • Performs administrative duties within clinical practice area as assigned.
Work Schedule: Monday - Friday; 7:30 am - 4:00 pm or 8:00 am - 4:30 pm or 8:30 am - 5:00 pm

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education. (TRANSCRIPTS REQUIRED).
  • Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico) or the District of Columbia. (COPY OF LICENSURE REQUIRED).
  • English Language Proficiency: Must be proficient in spoken and written English as required by 38 U.S.C.7402(d), and 7407(d).
Preferred Experience: Post Graduate Year -1 Residency; board certified geriatric pharmacist

Grade Determinations:

GS-12

(a) Experience or Education. In addition to the basic requirements, candidates must meet1.
1 year of experience equivalent to the next lower grade level, or
2. Completion of an ACPE-accredited Pharm.D. program.

Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
1. Knowledge of professional pharmacy practice.
2. Ability to communicate orally and in writing to both patients and health care staff.
3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

GS-13
(a) Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
(b) Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.

Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. Pharmacists assigned to this position must demonstrate the following KSAs:
1. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
2. Expert understanding of regulatory and quality standards for their program area.
3. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
4. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
5. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

The full performance level of this vacancy is GS-13.
