The senior social worker is responsible for the overall function of the Medical Foster Home Program at the Alaska VA Healthcare System in Anchorage, AK. The incumbent oversees program development, recruitment, and development of the foster homes, recruitment and admission of veteran residents, case management, development of policies and care processes, and coordination with HBPC interdisciplinary treatment teams.

  • Works closely with the VHA Public Affairs Office to market the program internally and externally community groups and agencies to promote the foster care program.
  • Utilizes national Directive, application forms, and inspection tools to screen and assess applicants wishing to become a MFH Caregiver which includes approving and disapproving MFH Caregivers in coordination with the approving official.
  • Provides oversight during monthly unannounced visits to the MFH and addresses concerns of Veterans and caregivers that supports the MFH model of care.
  • Establishes intake processes and oversight responsibilities and maintains a record-keeping system for the program.
  • Provides training to caregivers, students, residents, interns, and fellows from other disciplines regarding the advantages and disadvantages of a medical foster home.
  • Prepares/submits reports as required to the national program office and local and VISN leadership.
  • Direct supervision of staff as assigned by the Geriatrics and Extended Care service.

Veteran Screening/Assessment.
  • Receives/completes consults from interdisciplinary teams and other social workers on complex, difficult cases, using advanced practice skills and expertise. Incumbent also provides consultation to referral sources and is responsible for development and maintenance of professional standards of practice and educating others regarding such.
  • Documents in the medical records utilizing paper/electronic formats, including psychosocial assessments, diagnostics, treatment, progress notes, follow up and referrals.
  • Interviews/conducts psychosocial assessments; interviews veteran and family members and/or significant others to establish facts about the veteran's unique situation; presenting problems and their causes, and the impact of such problems on the veteran's functioning and health as a part of a comprehensive psychosocial assessment; assesses high risk factors, acuity and need for services.
  • Develops an assessment of the veteran in collaboration with the interdisciplinary treatment teams, the veteran, family members and significant others.
  • Reviews all data, subjective and objective, and makes a clinical assessment.
  • Uses advanced clinical training, insight, and experience to interpret data and to identify viable treatment options, including admission to a Medical Foster Home.

Treatment Planning.
  • Develops psychosocial treatment plans with HBPC interdisciplinary team, including goals for psychosocial clinical treatment.
  • Using available resources and the initial assessment of the veteran's likelihood to accept differing types of assistance, makes decisions regarding use of VA/non-VA services/referrals.
  • Provides consultation and makes recommendations to interdisciplinary team on course of treatment.

Evaluation and Management.
  • Establishes a continuing relationship with the foster home caregiver and veteran, and involved family members, evaluating progress towards goals and adjusting the treatment plan as appropriate.
  • Meets with medical foster home caregiver, veteran, and/or family members to assess accomplishments and re-establish goals.
  • Monitors progress, maintains documentation, ensures diagnosis and treatment of clinical disorders, and shows discretion in decision to adjust treatment.
  • Makes recommendations for care and changes to interdisciplinary treatment plan.
  • Provides the patient/caregivers and family members with ongoing supportive counseling.
  • Assists patient/caregivers in coping and dealing with the loss and grief experienced in chronic illness, terminal illness and death.
Advocacy.
  • Acts as an advocate when it serves the best interest of the veteran/family.
  • Acts as an advocate for the VA and need for services in specific geographical areas.

Crisis Intervention.
  • Makes rapid assessments and developing crisis management plans to maintain the veteran in a medical foster home.
  • Ensures the interdisciplinary team is kept informed of any concerns with the patient and/or caregiver.
Service line support.
  • Accepts additional and special assignments or duties that are necessary to meet program and all GEC service line goals and/or customer needs.

Qualifications

To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement.

Basic Requirements:

  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f)
  • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
  • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).

Grade Determinations:
GS-12 Program Coordinator Social Worker
  • Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
  • Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
  • Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
    • (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring
    • (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
    • (c) Ability to supervise multidisciplinary staff assigned to the program.
    • (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
    • (e) Ability to provide training, orientation, and guidance within clinical practice

Preferred Experience: At least 2 years of Social Work experience. Has specialized knowledge of practice typically related to dementias, chronic illnesses, traumatic brain injuries and care for the older adult and caregiver.

The full performance level of this vacancy is GS-12. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-12.
