This job opportunity announcement (JOA) will be used to fill one (1) full-time and permanent Clinical Pharmacy Specialist (Mental Health), GS-0660-13, vacancy at the Mt. Home, TN VA Medical Center, with Pharmacy Service.
The Mental Health Clinical Pharmacy Specialist (CPS) is a licensed pharmacist having completed a pharmacy practice residency and preferably a psychiatry specialty residency or other equivalent clinical experience and is encouraged to hold board certification in psychiatric pharmacy (BCPP). The incumbent is responsible for managing clinical pharmacy support in the outpatient mental health setting, and also provides support to clinical staff with regards to mental health. The majority of the CPS's time will be spent providing comprehensive pharmaceutical care to patients in this practice area and in clinical oversight of clinical staff pharmacists practicing in this area as related to mental health. The incumbent will also serve as a pharmacist provider assisting physicians, residents, interns, nurse practitioners, and physician's assistants in active mental health pharmacotherapy management of individual patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services.
Major duties include but are not limited to:
Develops and maintains an in-depth knowledge of pharmacy practice appropriate to mental health.
Develops professional relationships with non-pharmacy professional personnel.
Oversees clinical staff pharmacists' management of patient therapy, as they relate to mental health.
Reviews and evaluates prescribed mental health pharmacotherapy daily for appropriateness, effectiveness, cost-benefit, adverse drug reactions, allergies, drug-drug and drug-disease interactions, and communicates and documents recommendations to providers and pharmacists as necessary.
Appropriately renders pharmaceutical care to mental health patients and documents care in the medical record.
Acts as a pharmacotherapy expert within mental health and provides expertise to other providers, pharmacists, nurses, and other disciplines as appropriate.
Designs and performs relevant drug use evaluations under direction of the Associate Chief, Pharmacy - Clinical & Educational Services.
May initiate and/or participate in research studies in practice areas.
Is encouraged to hold board certification in an appropriate pharmacy specialty area (BCPP).
All inpatient/outpatient medication orders are checked to include patient information, correct medication, dose, route, and frequency of administration. Orders are checked for patient allergies, drug interactions, incompatibilities, duplication, and priority.
Identifies, interprets, and resolves prescribing and dispensing issues.
Dispenses controlled substances with accuracy according regulations.
Supervises and directs pharmacy support staff to achieve dispensing goals of the service.
Supervises and trains pharmacy residents and students in mental health rotations as well as pharmacy staff regarding mental health pharmacotherapy. Evaluates students based on performance, presentations, and examinations. Provides didactic training on mental health drug therapy to pharmacists, students, pharmacy residents, and other professional healthcare staff. Maintains organized, structured, clinical rotations for pharmacy students and residents.
Participates in formal and informal educational programs of the service.
Avails himself/herself of robust educational opportunities, and regularly participates in such activities.
Leads clinical staff pharmacists in providing pharmaceutical care to mental health patients.
Actively assures that policy and procedures of the Service and medical center are followed.
Assists the Chief of Pharmacy, the Associate Chief of Pharmacy - Clinical & Educational Services, and pharmacy supervisors in developing policies and procedures.
Represents pharmacy on assigned committees as needed.

Qualifications
Health Professions Trainee applicants pending the completion of the grade experience or licensure requirement may be referred and tentatively selected but may not be hired until all requirements are met.
Creditable Experience
(1) Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board.
(2) Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination).
(3) Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination.
(4) Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service.

Today the sole degree is Doctor of Pharmacy.)(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.

Clinical Pharmacy Specialist. The clinical pharmacy specialist (CPS) functions at the highest level of clinical practice, works independently under their scope of practice as defined by the individual medical center to directly care for patients. A CPS plays a defined role in budgetary execution and serves as a mid-level provider who functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.

b. Expert understanding of regulatory and quality standards for their program area.

NOTE: Participation in the seasonal influenza program is a condition of employment and a requirement for all Department of Veterans Affairs Health Care Personnel (HCP). It is a requirement that all HCP to receive annual seasonal influenza vaccination or obtain an exemption for medical or religious reasons. Wearing a face mask is required when an exemption to the influenza vaccination has been granted. HCP in violation of this directive may face disciplinary action up to and including removal from federal service. HCP are individuals who, during the influenza season, work in VHA locations or who come into contact with VA patients or other HCP as part of their duties. VHA locations include, but are not limited to, VA hospitals and associated clinics, community living centers (CLCs), community-based outpatient clinics (CBOCs), domiciliary units, Vet centers and VA-leased medical facilities. HCP include all VA licensed and unlicensed, clinical and administrative, remote and onsite, paid and without compensation, full- and part-time employees, intermittent employees, fee basis employees, VA contractors, researchers, volunteers and health professions trainees (HPTs) who are expected to perform any or all of their work at these facilities. HPTs may be paid or unpaid and include residents, interns, fellows and students. HCP also includes VHA personnel providing home-based care to Veterans and drivers and other personnel whose duties put them in contact with patients outside VA medical facilities.
